Abstract
Regorafenib 是一種口服多激酶抑制劑,臨床上用於治療難治性轉移性結腸直腸癌、肝細胞癌和胃腸道間質瘤。由於此藥物之水溶解度低,本研究嘗試以矽奈米粒子改變其藥物動力學特性,同時測試藥物在載體中之穩定性。矽奈米粒子是近年來藥物製劑的研究趨勢之一,由二氧化矽組成的奈米粒子因其生物相容性佳,且成本相對較低,因而被廣泛探討其於製藥、影像醫學等相關領域之應用。
在本研究中,使用一種簡單的合成方法進行奈米粒子製備,官能基包含硫醇基(SH) 和胺基(NH2)。觀察了不同官能基團比例、矽烷濃度、反應時間和溫度、離心速度對奈米粒子結構之影響。利用本研究中所開發的方法,可以有效製備出高包覆率(90-98 %)及良好回收率(60-80 %)之雙功能矽奈米粒子,在評估反應過程中之各個參數對於粒子的影響後,發現以25℃反應、M:A=29:1 之有機相進行製備時,能得到相對較多的奈米粒子量,粒徑大小分佈均一,介於130~170 nm 之間,表面電荷介於 +45-55mV,並且能於4 ℃之環境穩定存放至少4 週,具備良好的安定性。
安定性試驗結果發現,Regorafenib 在甲醇溶液中至少可維持8 週的安定性,矽奈米粒子水溶液也在4 ℃與20 ℃表現出良好的安定性。而在大鼠血漿中,Regorafenib 僅能維持24 小時之安定,預先進行前處理之血漿後則能於4 ℃或20 ℃維持至少48 小時之安定。在藥物動力學研究中,以Sprague-Dawley 大鼠作為模型動物,採用口服管餵和靜脈注射給藥。發現奈米粒子組之有較高的血中濃度,且於不同劑量間皆有相同的趨勢,有較好的藥物吸收效果,曲線下面積也提升了2.6 倍至5.2 倍,而藉由奈米粒子給藥,也能夠降低食物造成藥物動力學之影響。在安全性評估中發現,奈米粒子組皆有白血球相關數值上升之趨勢,也會降低藥物對肝臟相關數值的影響,體重監測則沒有明顯差異。總體而言,Regorafenib 於矽奈米粒子並不會產生明顯的生物毒性。
綜上所述,本研究開發了裝載Regorafenib 的雙官能基矽奈米粒子,並完成了其物化性質、藥物包覆率、安定性、藥物動力學和安全性等方面的測試。這些發現證明了矽奈米粒子的藥物動力學潛力,或許未來能作為化療藥物遞送的另一種選擇。
