Abstract
慢性腎臟疾病隨著疾病的進展會逐漸惡化,其成因相當多樣化,包含糖尿病、高血壓、自體免疫性疾病等等。腎小管、腎小球或兩者兼有的損傷都可能導致慢性腎臟疾病的發生,而纖維化、細胞損傷和微血管損傷則是其進展惡化的重要因素。慢性腎臟疾病影響群體眾多,隨著疾病的惡化,相關併發症如心血管疾病和貧血等也會隨之出現,因此阻止慢性腎臟病的進展並提供有效治療是改善其預後的關鍵目標。
磷酸二酯酶在分解環磷酸腺苷、環磷酸鳥苷或兩者方面發揮著至關重要的作用,其作用與影響具體取決於其亞型之不同。其中一個較廣為人知的亞型為第五型磷酸二酯酶,主要存在於陰莖組織和呼吸道平滑肌細胞中。第五型磷酸二酯酶的抑制劑能夠有效阻止平滑肌細胞中的環磷酸鳥苷降解,因此對治療勃起功能障礙和肺動脈高壓具有療效。最近的研究發現,第五型磷酸二酯酶抑制劑可能也對慢性腎臟疾病有治療效果。先前的研究顯示,第五型磷酸二酯酶抑制劑可調節腎血流,增強糖尿病腎臟的微循環,並通過減少腎小球基質的積聚緩解腎小球硬化。因此,我們的研究旨在更深入探索第五型磷酸二酯酶抑制劑的潛在腎保護作用以及它們減緩腎臟疾病進展的能力。
我們在研究中評估了第五型磷酸二酯酶抑制劑伐地那非在阿黴素所誘導的局部節段性腎小球硬化症小鼠模型中的治療潛力。局部節段性腎小球硬化症是成人末期腎病的主要原因之一,其特點是腎小球足細胞的減少和損傷,進而造成疾病的持續惡化。基於先前的證據表明,第五型磷酸二酯酶抑制劑可能減輕腎小球硬化的嚴重程度並預防足細胞損傷,我們的研究旨在確定其抑制劑是否可以保護並阻止局部節段性腎小球硬化症的進展。我們的研究結果顯示,伐地那非治療具緩解局部節段性腎小球硬化症小鼠蛋白尿、腎功能障礙和高膽固醇血症惡化的效果。伐地那非治療與在組織學分析中觀察到的腎小球硬化和腎間質損傷嚴重程度的減輕相關聯。此外,伐地那非治療不僅緩解了阿黴素誘導的足細胞損失,還保護了腎小球中微血管的內皮細胞。伐地那非治療還改善了腎膈細胞間質與鮑氏囊基底膜中膠原纖維的積聚,並能抑制轉化生長因子-β 誘導纖維母細胞轉分化成肌成纖維細胞的能力。我們的研究顯示,第五型磷酸二酯酶抑制劑具有腎保護作用,為其作為腎病症候群的治療靶點提供了新的見解。
