Abstract
笑容在人類顏面表情中扮演很重要的角色,一個美麗的笑容能夠使人更加自信並且為整體面容加分。隨著時代對於美的概念的著重,病人接受矯正治療不再僅是對於齒列咬合及功能的需求,更希望改善外觀尤其是笑容的美觀度。本研究目的希望藉由定量測量法來了解不同咬合型態在矯正治療前及治療前後笑容的差異,以及有哪些側顱因子影響其笑容。

本研究屬於回顧性研究,受試者皆為2011年至2013年於台北醫學大學齒顎矯正科接受矯正治療並且治療完成之病人。106位符合條件之受試著納入本研究中,依據(I)前牙水平覆蓋關係及(II)前牙垂直覆蓋關係這兩項分類,將受試者分成三組,測量其治療前後側顱影像及正面笑容相片做比較。利用ANOVA分析各分類中三組間笑容變相之關係;利用Paired t檢定分析治療前後笑容及側顱變相的差異性;利用多元線性回歸分析影響笑容變相之側顱因子。

在前牙水平覆蓋關係的分類中,治療前各組間笑容差異性較大,且治療前後笑容的改變均有統計上顯著的差異。在前牙垂直覆蓋關係的分類中,無論是治療前或治療前後笑容變相的差異均比水平關係分類來的少,所受到側顱變相影響的笑容因子也較少。因此,前牙水平覆蓋的差異是最主要影響笑容的因素。在矯正治療的臨床運用上,適當前牙水平覆蓋關係的建立是改善笑容的關鍵。
