Abstract
台灣低收入族群有逐年增加的趨勢。低收入族群有較高的疾病率和住
院率,且預後情況較差。國內目前尚無為探討低收入與外科術後重大併發症及死亡率之研究,國外研究亦局限於單一手術或同一類手術,且有諸多限制。因此,本研究將探討外科手術病人低收入與否,與術後30天內包括併發症、死亡率手術不良結果。
本研究使用全民健保研究資料庫 2004 -2010 年全國外科手術特殊需求檔,擷取接受全身與半身麻醉接受住院手術病人,並串連這些人 2002-2010 年醫療就醫記錄進行分析。採用傾向分數配對的巢式世代研究,比較組間術後 30 天內併發症與死亡率勝算之比值。配對的因子包括個案年齡、性別、手術前疾病史、手術類別、麻醉方式與醫療機構特性(醫院所在地都市化程度、教學層級)。進一步再利用手術類別與醫療院所特性之分層分析,探討低收入與手術後 30 天內死亡率之關係。本文之所有的統計分析,皆以 SAS9.1 版軟體來進行分析。
結果顯示:2004 - 2010 年研究對象,低收入手術病人共有 76,303 人,非低收入手術病人共有 3,040,597 人,以 1:1 的方式配對產生低收入組與非低收入組各 76,303 人。低收入外科病人的男性比例比女性多(52.6%對47.4%)。相較於非低收入的病人,低收入手術病人術後 30 天內之死亡率的危險比為 1.45。而且術後 30 天內併發症與低收入有顯著的關係,如中風(OR=1.98)、肺炎(OR=1.81)、敗血症(OR=1.74)、急性腎衰竭(OR=1.34)、術後急性出血(OR=1.33)、術後深部傷口感染(OR=1.29)。其他相較於非低收入病人,與低收入病人呈現顯著相關還包括有住院天數過長(OR=1.37)、術後轉入加護病房(OR=1.28)與住院醫療費用過多(OR=1.19)。低收入病人在非教學醫院(OR=3.17)、手術量<500 之醫院(OR=3.24)、醫師總人數<20 人之醫院(OR=2.41)、醫事人員<100 人之醫院(OR=2.46)、床位數<100 之醫院(OR=2.27)及沒有加護病房之醫院(OR=3.75)術後 30 天內死亡率有顯著上升。
本研究結論顯示,低收入是術後 30 天內併發症與死亡率的危險因子,且就醫之醫療院所為非教學醫院、醫院床位與手術量少、沒有加護病床、醫師與專業人員數量少、則是與術後 30 天內死亡風險顯著相關。
