Abstract
背景:纖維化-4(FIB-4)指數和 AST/ALT 比率(AAR)是肝病的明確生物標記物,其水平升高與晚期肝病(包括肝硬化)相關。本研究結合基因組分析、轉錄組關聯研究 (TWAS) 和多基因風險評分 (PRS),整合 FIB-4 指數和 AST/ALT 比率值,以研究 HBV 陽性個體肝纖維化和肝硬化預測背後的遺傳和轉錄組結構。

方法:對於 FIB-4 指數研究,使用來自台灣人口的數據進行綜合分析,以確定與肝纖維化相關的關鍵基因變異。相較之下,AST/ALT 比率研究採用了由台灣和歐洲人口組成的跨血統隊列。在進行全基因組關聯研究 (GWAS) 之後,我們應用 TWAS 探索與肝臟酵素調節相關的遺傳因素,然後透過 PRS 計算來預測肝硬化。兩項研究進行了通路富集分析,以確定與肝病進展相關的關鍵生物學通路。

結果:在FIB-4指數研究中,我們確定HBS1L、HLA-A、ITPR3、MPIG6B和ABHD16A是與肝纖維化相關的重要基因。研究發現,免疫相關途徑在肝纖維化的進展中發揮著至關重要的作用。在跨祖先 AST/ALT 比率研究中,CHUK-CWF19L1-ERLIN1 基因簇、PIK3R2、EGFL8 和 SH2B3 被確定為肝功能和發炎的重要調節劑。通路分析強調免疫訊號和細胞訊號是肝病涉及的關鍵生物過程。對於肝硬化預測,基於 FIB-4 和 AST/ALT 比率的 PRS 最初顯示出適度的預測能力,但加入額外的人口統計協變量顯著提高了預測準確性。 PRS 模型顯示男性肝硬化的預測能力比女性更強。

結論:本研究對與肝病生物標記相關的遺傳和轉錄組因素進行了全面分析,包括 FIB-4 指數和 AST/ALT 比率。與免疫反應和細胞訊號傳導相關的關鍵途徑為肝纖維化的進展提供了有價值的見解。雖然單獨的 FIB-4 和 AST/ALT 的 PRS 顯示出中等的預測能力,但透過納入臨床協變量可以增強其性能,特別是改善男性個體肝硬化的風險分層。
