Abstract
骨關節炎(Osteoarthritis, OA)又稱為退化性關節炎,致病原因與退化、老化、外傷或關節過度使用有關。目前治療方法多針對軟骨組織注射葡萄糖胺及透明質酸,而嚴重之患者仍需接受人工關節置換手術的治療。本研究首先以發炎前驅物質介白質-1b(Interlukin-1b, IL-1b)與腫瘤壞死因子-a(Tumor necrosis factor-a, TNF-a)建立類關節炎之細胞模式,誘發人類關節軟骨細胞株hPi 產生發炎反應,並利用血小板富含血漿(Platelet- rich plasma, PRP)促進軟骨細胞再生,以建立關節炎的治療模式。實驗結果發現,IL-1b與TNF-a的誘導下,人類軟骨細胞株hPi 之增生能力明顯受到抑制,而利用PRP 處理後,hPi 之增生速率則有明顯之上升。利用RT-PCR 偵測基因現,軟骨細胞主要分化特異性基因SOX-9、type II collagen、Aggrecan 表現量會因IL-1b與TNF-a之添加而受到抑制,而PRP 處理後,其軟骨特異性基因之表現明顯增加。由體外膠原蛋白三維立體空間培養之實驗結果顯示,相較於IL-1b與TNF-a處理組,PRP 處理組其形成之軟骨組織保水度較佳且形狀較完整;組織切片結果亦發現,處理PRP 後,hPi 周圍有明顯的陷窩(lacunae)產生,利用免疫化學染色法與Alcian blue 染色法發現,在PRP 之組別中,其第二型膠原蛋白組織染色與蛋白醣沉積有較明顯之表現。綜觀上述之實驗結果顯示,PRP 具有改善關節炎所引起之軟骨組織退化及促進軟骨再生之潛力。
