Abstract
本實驗室先前的研究顯示尿中的D-乳酸有潛力成為腎損傷的新指標之一,根據文獻指出metformin (Met)可以降低體內D-乳酸前驅物—甲基乙二醛(methylglyoxal, MG)的含量,本實驗室先連續七天口服給予囓齒類動物Met後,再同時以腹腔注射給予gentamicin (GM)或馬兜鈴酸(aristolochic acid, AA)誘導腎損傷,實驗結果顯示Met可降低腎臟中MG含量並改善腎損傷,故本研究欲觀察在GM及AA動物模式下,Met對於組織中D-乳酸及MG之影響。實驗結果顯示,以GM (150 mg/kg/day, i.p., 6 days)誘導腎損傷,血中之MG顯著性增加(173.42 ± 7.82 vs. 410.37 ± 63.15, p < 0.01);以AA (5 mg/kg /day, i.p., 8 days)誘導急性腎損傷,尿中D-乳酸顯著性增加(0.72 ± 0.08 vs. 3.33 ± 0.79, p < 0.01),皆與生化值檢驗值趨勢相同,故血清中之MG與尿中D-lactate有潛力可分別作為評估GM和AA腎損傷之新指標,但不同藥物誘導腎損傷之機轉對於MG和D-乳酸之影響尚未釐清,還需進一步研究與探討。此外pre-treat給與Met可分別降低兩種動物模型中血清中之MG (410.37 ± 63.15vs. 200.42 ± 9.78, p < 0.01)和尿中之D-乳酸(3.33 ± 0.79 vs. 0.90 ± 0.17, p < 0.01),改善腎功能;Met用於腎損傷時需監控血中L-乳酸濃度,以達改善腎功能但不會乳酸中毒之效果。
