Abstract
背景:台灣護理學會自1992年開始推動「基層護理人員臨床專業能力進階制度」,至今已有二十餘年。然而,許多臨床護理人員對於參加進階制度的動機相當薄弱,也缺乏進階的意願,縱使醫院推行獎勵措施,仍無法使護理人員積極參加。此外,目前對於進階制度的參加動機與意願性也缺乏研究探討。
目的:本研究之目的乃探討基層護理人員參加臨床專業能力進階之成效與相關影響因素。
方法:本研究以結構式問卷為研究工具,收案時間為105年2月1日至105年3月30日。在台灣北部五所已實施專業能力進階制度之醫療院所進行收集,共發出500份問卷,回收有效問卷415份,回收率為83%。
結果:研究結果發現,臨床基層護理人員以20至25歲 (28.9%)、未婚 (58.8%)、專科學歷 (52.8%)、職級為N1 (36.4%)之護理人員為最多數,於目前醫院服務年資平均為7.13年,護理總年資平均為9.86年。個人因素中的服務年資對於進階壓力(F =3.90 , p= 0.03)以及進階滿意度(F = 3.67 , p= 0.02)存在顯著差異,而組織因素的進階制度則對於護理人員的進階動機(F = -10.97 , p= 0.00)與進階滿意度(F = -12.17, p= 0.01)存在顯著差異。臨床基層護理人員進階滿意度之重要影響因素,為個人因素中「教育程度」(β = -.10, p < 0.05)、組織因素中「進階制度」(β =.18, p < 0 .01)、「獎勵制度」(β =.21, p < 0.01)、「單位工作負荷」(β =.14, p < 0.01)、以及「進階動機」(β = .27, p < 0.001)。
結論:研究結果將提供醫院管理者及護理行政主管未來輔導護理人員參加專業能力進階制度參考,並作為修訂、推動及宣導進階制度之依據。
