Abstract
肥胖為熱量攝取不平衡造成體內脂肪堆積過多,一般皆伴隨著脂肪細胞的增大或數目變多。成熟的脂肪細胞會分泌leptin及adiponectin等脂肪細胞激素調控能量之代謝。肥胖者血中一般皆有較高leptin及較低adiponectin,且此狀況與肥胖相關疾病之發生密切相關。脂肪組織會因組織擴張導致缺氧而誘發缺氧誘發相關因子的產生,包括HIF-1α與aryl hydrocarbon receptor nuclear translocator (Arnt) heterodimer的形成,進而可影響組織內血管新生因子、glucose transporter 1 (GLUT1)及leptin等的基因表現。Arnt亦可與aryl hydrocarbon receptor (AhR)形成heterodimer,而影響細胞色素P450的表現,並協助體內解毒酵素系統的代謝。研究指出,增加成熟脂肪細胞中AhR的活性,有助於降低脂肪細胞內脂質生合成然而目前尚未有針對肥胖者脂肪組織中Ahr及Arnt的表現情形進行探討。
,因此本研究將分析肥胖者者皮下脂肪組織及內臟脂肪組織中缺氧誘發相關因子及血中leptin與adiponectin的表現情形。受試者為119名於臺北醫學大學附設醫院體重管理中心接受袖狀胃切除減重手術BMI大於35的肥胖受試者,年齡20-50歲。經簽署同意書後在手術前取得受試者基本資料及血液生化數值,手術時取空腹血液分析血中leptin、adiponectin濃度及皮下、腹部脂肪組織進行H&E染色、mRNA表現量分析。結果發現台灣地區重度肥胖 (BMI≥35)的男性及女性其腰圍、臀圍、體重、脂肪重、C-反應蛋白、leptin皆隨BMI增加而增加。男性及女性之leptin及C-反應蛋白濃度的增加亦隨腰圍、臀圍、體重、脂肪重及BMI等增加,且leptin濃度增加,C-反應蛋白亦隨之增加。男性之皮下脂肪組織AhR mRNA表現量皆隨腰圍、臀圍、體重、脂肪重及BMI上升而隨之降低的,但皮下脂肪組織Arnt僅依臀圍及體重的增加而降低。而女性之leptin濃度增加,血糖、糖化血色素則是隨之下降,但缺氧相關因子mRNA表現則是與體位間皆無顯著相關性。總言之,男性重度肥胖者之皮下脂肪組織AhR及Arnt mRNA表現情形與體位間呈負相關性,而在女性重度肥胖者則無此現象。
