Abstract
關節軟骨 (AC) 是一種無血管且無神經的結締組織,具有有限的自我修復能力,因此極易受到創傷和退化性關節疾病(如骨關節炎 (OA))的損傷。對有效再生策略的需求導致人們對光生物調節 (PBM) 的興趣日益濃厚——這是一種非侵入性、無藥物且安全的方法,可透過光生物效應增強細胞活性並減少發炎。儘管如此,PBM 的治療效果高度依賴波長、能量密度和治療方案等參數。本研究首先回顧了 PBM 在軟骨再生中的應用,重點介紹了常用的雷射:紅光雷射(600-660 nm,0-10 J/cm²)、GaAIAs 雷射(800-880 nm,10-50 J/cm²)和 Nd:YAG 雷射(1064 nm,高達 200 nm/cm)和 Nd:YAG 雷射(1064 nm,Lcm)在體外的應用研究中。 PBM 與軟骨組織工程 (CTE) 的結合也顯示出協同效應,可改善基於支架的修復效果。
在體外階段,我們探索了 PBM 與藻酸鹽基支架相結合,以促進軟骨細胞衍生的體外軟骨再生生物反應。在此,我們利用不同濃度的 CaCl₂ 製備了藻酸鹽基支架,以實現最佳的機械性能、生物相容性和生物降解性。在二維環境中,我們研究了 2.5 至 10 J/cm² 能量密度範圍內近紅外線 (NIR) 雷射輻照 (830 nm) 對軟骨細胞活力和增殖的影響。最終確定了 3% w/v CaCl₂ 和 5 J/cm² 的最佳條件,以建立藻酸鹽支架並促進二維和三維培養中的軟骨細胞生長。多次PBM治療(830 nm,5 J/cm²)顯著增強了藻酸鹽支架中的軟骨細胞增殖和糖胺聚醣生成,其中每四天一次的方案最為有效。掃描電子顯微鏡顯示PBM誘導了分泌顆粒的形成,這對應於細胞外囊泡(EV)釋放的顯著增加。
此外,在OA大鼠模型中評估了PBM與藻酸鹽包埋的ADSC衍生EV(ADSC-EVs/Alg)的聯合治療潛力。優化的注射劑型(10%藻酸鹽,1:9 Alg:EV比例)在體內外均表現出良好的流變學和生物學特性。 PBM增強了細胞反應,其與ADSC-EVs/Alg的聯合使用進一步改善了體外療效。儘管如此,體內實驗結果顯示,所有治療組的軟骨修復程度相似,且合併治療並未帶來顯著優勢。這些發現表明,雖然 PBM 和 ADSC-EVs/Alg 單獨使用都有效,但它們的同時使用並不能為 OA 中的軟骨再生提供額外的治療益處。
