Abstract
研究目的:輕微疾病的不適症狀會影響病人的日常生活,降低其生活品質,而低都市化地區的醫療選擇少,兼之交通便利性低,使當地居民的醫療可近性較低。本研究分析老年居民輕微疾病管理現況,藉此瞭解民眾現行管理策略與目前醫療資源利用的概況,並進一步建立低都市化地區老年居民輕微疾病管理模式,從中釐清當地居民在遭遇輕微疾病事件時的思考脈絡與後續行動。
研究方法:本研究以焦點座談的形式,利用半結構式訪談大綱訪問當地居民管理輕微疾病的經驗;大綱包括四個輕微疾病情境,讓受訪者有更具體的討論。本次收案採立意取樣,於低都市化鄉鎮張貼招募海報,供年滿65歲且過去有管理輕微疾病經驗的當地居民自願報名。每場焦點座談約6-12位受訪者,每場約60分鐘。焦點座談皆進行錄音,並打成逐字稿作質性分析。本研究利用紮根理論分析並建立低都市化地區老年居民輕微疾病管理模式。
結果:本次研究共計於5個縣市內的8個鄉鎮舉辦焦點座談,依理論飽和於第8場時結束收案。最終總計47名受訪者,平均年齡為76.1歲,分別為18位男性、29位女性。受訪者在遇到輕微疾病時,會先以內建的知識與經驗來觀察目前身體的狀況。如果自覺對輕微疾病的資訊不足,可能會尋求外界資源以獲得所需的醫藥資訊,資料來源包括醫療人員衛教、親朋好友交流,或講座、書籍等。受訪者管理輕微疾病的認知與行為模式為整體流程中之重要因素,其來自於背景的知識與經驗,且直接影響病人後續的管理策略。行動可分為醫療尋求與自我照護。病人在採取這些行動後所得到的成效好壞,會影響後續面對輕微疾病的管理策略與態度。每次的輕微疾病管理經驗都會協助個人輕微疾病管理模式的形成。
結論:輕微疾病在日常生活中很常見,因此受訪者會從過去的生活經歷中,摸索出一套個人的心得與行為模式,使受訪者對自己的想法較執著,作出決策的過程也很直接。在整體管理輕微疾病流程中,民眾會用自己的角度來解讀症狀發生的緣由與不同治療策略的好壞,憑自身經驗,常慣性地選擇非藥物自我照護法。此外,正面與負面的治療經驗會直接影響病人後續採取積極或消極的管理策略。專業醫療人員可利用此模式瞭解病人目前的疾病管理狀態,並進一步辨識可以介入的步驟,進行有效地溝通。未來研究可深入釐清民眾錯誤觀念的成因,與改正病人錯誤自我照護方法的策略。
