Abstract
巴金森氏症(Parkinson’s Disease, PD)是一種常見的神經退行性疾病,主要影響運動控制,導致靜止性震顫、動作遲緩、肢體僵硬、步態不穩等症狀。步態和平衡障礙是巴金森氏症早期的重要特徵,雖然在外觀上不明顯,但透過精密測量可檢測異常變化。研究顯示,步行速度減慢、步態變異性增加與跌倒風險密切相關。然而,傳統臨床評估主要依賴醫師觀察與個案主觀陳述,難以全面掌握早期功能變化。隨著穿戴式裝置技術的進步,這些裝置結合加速度計與陀螺儀,可精確記錄步態與平衡數據,提供更客觀且可靠的跌倒風險評估。
本研究採用橫斷式研究設計,於新北市雙和醫院神經內科門診招募50 至80歲的早期巴金森氏症個案,並以社區健康長者作為對照組,透過非隨機抽樣法選取受試者。研究運用穿戴式裝置測量步態與平衡參數,並結合國際跌倒效能量表(Falls Efficacy Scale International, FES-I)及柏格氏平衡量表(Berg Balance Scale, BBS)進行資料收集。統計分析方法包括描述性統計、常態性檢定、組間差異分析與 Spearman 相關性分析,以探討 FES-I、BBS 與步態及平衡參數之關聯性;進一步採用二元邏輯斯迴歸、多重線性迴歸及逐步迴歸建立預測模型,所有統計檢定均以 p < 0.05 為顯著水準。
研究結果顯示,早期巴金森氏症組在多項步態與平衡參數上與對照組呈現顯著差異,包括行走速度降低、步態時間參數改變,以及靜態平衡時身體晃動與擺動速度增加。迴歸分析結果指出,特定步態與平衡參數可有效區分早期巴金森氏症個案與對照組,並具備良好的預測能力。此外,步態與平衡參數與 FES-I 及 BBS 分數之間呈現顯著相關,其中行走速度與平衡相關參數為影響跌倒恐懼與平衡功能的重要預測因子。
本研究結論顯示,穿戴式裝置可客觀量化早期巴金森氏症個案之步態與平衡變化,結合臨床量表有助於提升跌倒風險辨識與早期介入,進而降低跌倒相關併發症並改善生活品質。
