Abstract
研究背景
抗血小板藥物在預防栓塞事件上佔有重要的角色。然而,過去的研究對於clopidogrel 使用於預防再次中風是否優於aspirin尚沒有一致性的結果。Clopidogrel用於預防再次中風之療效及安全性至今仍是有待研究的議題。再者,80歲以上的老年人在過去探討clopidogrel用於預防再次中風的療效及安全性研究中僅佔極少數的比例。因此,clopidogrel使用於老年之缺血性腦中風病人之療效及安全性仍需再探討。另外,鮮少有研究探討在過去曾有缺血性腦中風之心肌梗塞病人使用ticagrelor合併aspirin相較於clopidogrel合併aspirin治療之療效及安全性。因此,在過去曾有缺血性腦中風之心肌梗塞病人使用ticagrelor合併aspirin治療是否優於clopidogrel合併aspirin目前仍是未知的。

研究目的
這個研究有以下三個主要的研究目的。
Aim 1: 比較缺血性腦中風病人使用clopidogrel及aspirin的療效及安全性。
Aim 2: 比較80歲以上的缺血性腦中風之病人使用clopidogrel及aspirin的療效及安全性。
Aim 3: 比較過去曾有缺血性腦中風之心肌梗塞病人使用ticagrelor合併aspirin及clopidogrel合併aspirin的療效及安全性。

研究方法
這是一個利用台灣衛生福利資料庫之全人口檔進行之回溯性世代研究。在Aim 1及Aim 2,研究納入在2009年1月1日至2018年12月31日間曾因缺血性腦中風住院的20歲以上(Aim 1)或80歲以上(Aim 2)的病人。納入研究之病人皆使用clopidogrel或aspirin以預防再次發生缺血性腦中風。在Aim 3,納入分析的病人為在2013年07月01日至2018年12月31日間曾因心肌梗塞住院的病人。這些人在心肌梗塞前皆曾被診斷患有缺血性腦中風的病史。Aim 3的病人在心肌梗塞出院時,需使用ticagrelor或clopidogrel作為預防再次發生心肌梗塞之藥物。此研究使用治療權重倒數機率的方法來平衡兩個分析的族群,並利用Cox比例風險模式來計算兩個分析族群間發生事件的風險。分析的項目包含重大心血管事件、缺血性腦中風、心肌梗塞、重大出血事件、顱內出血、及腸胃道出血。

研究結果
Aim 1: 共有71,101之缺血性腦中風病人納入這個研究,其中5,731人使用clopidogrel,65,370人使用aspirin。相較於aspirin,clopidogrel有顯著較低之重大出血事件(HR: 0.87; 95% CI: 0.83-0.92, p < 0.0001)及顱內出血(HR: 0.68; 95% CI: 0.61-0.75, p < 0.0001)的風險。然而,重大心血管事件、缺血性腦中風、心肌梗塞、及腸胃道出血的風險在兩組間則沒有顯著的差異。Aim 2: 納入此研究的80歲以上之缺血性腦中風病人共有15,045人,其中1,979人使用clopidogrel,13,066人使用aspirin。相較於aspirin,clopidogrel有顯著較低的重大心血管事件(HR:0.88; 95% CI: 0.82-0.95, p < 0.01)、缺血性腦中風(HR: 0.89; 95% CI: 0.83-0.96, p < 0.01)、重大出血事件(HR: 0.89; 95% CI: 0.80-0.99, p < 0.05)、及顱內出血(HR: 0.71; 95% CI: 0.56-0.90, p < 0.01)的風險。然而,在心肌梗塞及腸胃道出血的風險上,兩組則沒有顯著的差異。Aim 3: 此研究共納入1,691之曾有缺血性腦中風病史之心肌梗塞的病人。其中734人使用ticagrelor合併aspirin治療,而957人使用clopidogrel合併aspirin治療。相較於clopidogrel合併aspirin治療,ticagrelor合併aspirin治療有較低的重大心血管事件的風險(HR: 0.78; 95% CI: 0.62-0.97, p < 0.05)。然而,ticagrelor合併aspirin治療卻有較高的顱內出血的風險(HR: 3.34; 95% CI: 1.27-8.81, p < 0.05)。兩組間在心肌梗塞、缺血性腦中風、重大出血事件、及腸胃道出血的風險上皆沒有顯著的差異。

研究結論
在一般的缺血性腦中風病人,clopidogrel相較於aspirin可以顯著降低重大出血事件及顱內出血的風險。對於80歲以上之缺血性腦中風病人來說,clopidogrel相較於aspirin除了可以顯著降低重大出血事件及顱內出血的風險外,亦可以顯著降低重大心血管事件及缺血性腦中風的風險。故clopidogrel使用於缺血性腦中風病人,特別是80歲以上之缺血性腦中風病人,可被視為安全且有效的藥物。在過去曾有缺血性腦中風病史的心肌梗塞病人中,ticagrelor合併aspirin治療有顯著較低的重大心血管疾病的風險,然而其造成顱內出血的風險卻較高。因此,ticagrelor合併aspirin使用於這個族群必須密切監測其出血風險以確保其安全性。
