Abstract
攝護腺癌好發於50歲以上的男性,在臺灣十大癌症中高居第六名,且發生率在過去十年逐漸升高,是臺灣及西方國家不可忽視的疾病。砷為國際癌症研究署證實的一級致癌物,砷所誘發的氧化壓力可能是攝護腺癌的危險因子之一,但砷對於攝護腺癌的作用機制尚未明確。硒是人體一個重要且必須的微量元素,抗氧化作用為其主要的功能。有研究顯示體內硒含量增加能降低攝護腺癌的危險性。硒進入人體後會形成不同種類的硒酵素,其中穀胱苷肽過氧化酶為體內調控氧化還原反應的重要硒酵素。有研究顯示穀胱苷肽過氧化酶基因多形性會改變體內氧化壓力的狀態,與攝護腺癌風險有關,但穀胱苷肽過氧化酶之基因多形性與攝護腺癌的關係尚未明確。
本研究以醫院為基礎的病例對照研究,在臺大醫院泌尿科、臺北醫學大學附設醫院與萬芳醫院招募318位攝護腺癌病例,以年齡( 5歲)進行1:1配對318位對照組。研究對象皆由經過標準化訓練之訪員,利用結構式問卷進行面對面訪視,收集人口學變項、生活習慣與疾病史等資料,並收集血液及尿液樣本。利用白膜層(buffy coat)萃取DNA,使用聚合酶鏈鎖反應-限制片段長度多形性(polymerase chain reaction-restriction fragment length polymorphism, PCR-RFLP)進行分析穀胱苷肽過氧化酶之基因多形性。血漿利用感應耦合電漿質譜儀 (inductively coupled plasma-mass spectrometer)進行分析血漿硒濃度。利用高效能液相層析儀 (high-performance liquid chromatography)分離砷物種,連結氫化器(hydride generator)與原子吸收光譜儀 (atomic absorption spectrometry)測量尿液砷物種濃度。
高教育程度者相較於低教育程度者有較低的攝護腺癌危險對比值。父親、母親為外省人或原住民者相較於閩南人有較低的攝護腺癌危險對比值。在校正年齡、教育程度、父母親氏族後 GPX1 rs1050450 CT與TT基因型相較於CC基因型,有較低的攝護腺癌危險性危險對比值與95%信賴區間為0.49 (0.28-0.85)。高期級別相較於低期級別有較高的攝護腺特異性抗原濃度。另外也發現硒濃度越高攝護腺癌的危險對比值越低,呈顯著劑量效應關係。
