Abstract
背景:慢性阻塞性肺病(COPD)患者易併發衰弱(Frialty),導致活動耐力下降、生活品質惡化及死亡風險上升。因病情嚴重,此類患者往往難以執行傳統主動式肺復原運動,陷入衰弱加劇的惡性循環。震動訓練屬低負荷之被動運動,能誘發反射性肌肉收縮,為潛在的替代方案。目前研究多聚焦於全身震動(WBV),惟其仍受限於特定的禁忌症與生理門檻;局部震動(LV)則提供另一種更具部位針對性的介入路徑,有望填補 WBV 的應用缺口。然而,目前相關實證研究仍顯不足,亟需進一步探討 LV 對衰弱 COPD 患者之治療成效。
目的:本研究旨在將不同震動訓練系統應用於衰弱前期與衰弱的COPD患者,比較WBV與LV介入對其下肢功能、呼吸症狀、疾病嚴重度及生活品質之影響。
方法:本研究採平行組隨機對照試驗設計,依 1:1:1 分派受試者至WBV組、LV組與對照組,進行為期12週、每週兩次、每次20-40 分鐘的震動訓練。主要指標為下肢功能,次要指標包含呼吸症狀、疾病嚴重度與生活品質。評估時間點為於介入前(前測)、介入12週後(後測)及介入結束後4週(追蹤後測),共三次。資料分析採用廣義估計方程式(GEE)檢驗組間與時間的交互作用。
結果: GEE結果顯示在運動功能方面, WBV顯著提升股四頭肌最大肌力(B = 2.86, p = 0.02)並改善六分鐘步行距離(B = 29.77, p = 0.14);LV則在精準強化下肢上更具優勢,顯著提升臀大肌與股四頭肌之平均肌力(B = 2.23, p = 0.05)。在症狀緩解上,兩組介入後皆能顯著減輕主觀呼吸困難(mMRC中LV:p = 0.02;WBV:p = 0.01)。且兩種震動訓練具有顯著的延遲獲益與持續性:追蹤後測顯示 WBV 組在綜合症狀評估(CAT:B = -6.26, p = 0.01)、疾病嚴重度(BODE:B = -1.24, p = 0.04)及生活品質(CCQ:B = -6.93, p = 0.02)均達到顯著改善;LV組則僅在綜合症狀(CAT:B = -7.38, p = 0.03)呈現顯著的延遲效應。
結論:本研究證實WBV可用於提升患者整體功能,而 LV 雖在提升整體效果方面有限,但在強化特定肌群肌力與改善呼吸困難上展現特定效益。兩種震動均具潛力作為衰弱患者或居家復健情境中的選擇方案,對於無法承受WBV全身性物理衝擊之患者,LV可作為肺復原介入中精準強化下肢功能的有效替代策略。
