Abstract
研究背景:台灣的工安意外事件層出不窮,其產生的社會成本不可估計,影響範圍涵蓋工業、民生、經濟、交通等各個層面,引起社會的高度關注。其中倖存者所受到的心理衝擊不容小覷,且可能會對其造成長期的影響,近年來越多研究開始關注將心理健康明確地納入減少災害風險的必要性。但台灣少有針對工安意外事件倖存者心理衝擊進行的研究,且相關研究追蹤時間較短、皆使用問卷進行資料蒐集,無法得知意外事件對於所有倖存者造成長期心理影響以及變化。

研究目的:擬以八仙塵爆事件為例,以大型資料庫探討工安意外事件倖存者長期受到的心理衝擊。

研究方法:本研究採病例對照研究設計(Case-Control Study),使用台灣健保資料庫2015-2020年資料,分為每年門急診就診比率、每年住院就診比率及每人每年門急診就診次數三個類別,各以精神科、相關診斷及相關用藥三個面向分析八仙塵爆事件倖存者五年間心理衝擊的變化。

研究結果:1.每年門急診就診比率,病例組於精神科、相關診斷及相關用藥皆有較高的盛行率,相對風險為精神科(RR=11.66,95%CI=9.54-14.24)及相關診斷(RR=5.59,95%CI=4.60-6.80)以第二年最高,相關用藥(RR=4.42,95%CI=3.97-4.92)則是第一年最高。2. 每年住院就診比率,則是兩組於精神科皆無顯著差異,相關診斷前三年有顯著差異,相關用藥五年皆有顯著差異,五年相對風險皆以相關用藥最高,第一年相對風險高達122.41(95%CI=84.86─176.59)。3.病例組於精神科、相關診斷及相關用藥皆有較高的每人每年就診次數,相關診斷方面,為兩組於雙向情緒障礙症、反應性精神病及物質成癮五年皆無顯著差異,相關用藥則是情緒穩定劑五年皆無顯著差異。

結論:長期而言,倖存者會因工安意外事件而有較高的心理衝擊,甚至產生較高的相關醫療利用,可以做為提供當局計算重大公共安全事故潛在社會成本之參考依據。
