Abstract
根據統計近三十年來甲狀腺癌病患人數大幅上升,在所有甲狀腺癌中未分化型甲狀腺癌雖占少數,但惡性度高不易治療,這類型的癌症對於放射性碘和化療反應不良,死亡率高達40 %,五年存活率更是低於7 %。目前臨床上沒有針對未分化型甲狀腺癌病患有效之療法,本研究透過CCLE(Cancer Cell Line Encyclopedia)癌症細胞株資料庫分析各部位癌症生長因子與其受體表現,分析出甲狀腺癌細胞在EGFR、KDR(VEGFR-2)、PDGFRA相較其他癌症表現量較高且具有顯著差異,並且以FDA許可之TKIs (Tyrosine kinase inhibitors)抑制相對應生長因子受體訊息傳遞EGFR (Afatinib, IC50=8.5 µM),、KDR (Sutinib, IC50=7.5 µM)、PDGFRA (Imatinib, IC50=21.9 µM ),觀察這些藥物對於未分化型甲狀腺癌細胞SW579抗癌能力,提供參考依據給臨床使用這些標靶藥物在治療未分化型甲狀腺癌的可行性。此外本研究也為了探討這三個標的基因對於SW579調控生長有何影響,以基因剔除技術CRISPR/Cas9為基礎製作lentivirus濃縮液感染未分化甲狀腺癌細胞SW579,造成標的生長因子受體DNA被剪輯後觀察對未分化甲狀腺癌的抗癌能力。分析lentivirus對於基因剪輯效率及各基因在感染病毒後所產生之生長變化,發現基因產生受剪輯的SW579細胞生長較緩慢、惡性度降低、蛋白表現量變低等。本篇實驗已證實lentivirus在細胞層級達到抗癌效果,未來可以利用血液注射或是腫瘤內注射感染lentivirus發展活體試驗,具臨床應用價值不僅限於未分化癌治療,對於有抗藥性甲狀腺癌病患也是一大福音。
