Abstract
腦膜炎(Meningitis) 在成人中最常見的症狀為嚴重的頭痛,佔細菌性腦膜炎病例中90%。由於發炎的位置接近大腦和脊髓,因此常引起致命的危險,尤其新生兒腦膜炎的死亡率可高達10-30%。大腸桿菌(Escherichia coli) 是引起細菌性腦膜炎的常見病因。E. coli為人類腸道內的常在性細菌,在造成菌血症的同時,併發細菌性腦膜炎的機率提升。當生物組織受到刺激,引起發炎反應(Inflammation),攻擊排除有害刺激或病原體,並促進修復的保護措施。發炎反應主要由免疫細胞分泌相關的促發炎細胞因子:細胞激素(Cytokine) 和趨化因子(Chemokine)的參與。革蘭氏陰性菌外膜上的高度保留蛋白分子Outer membrane protein A (OmpA)是本研究的關鍵角色。本實驗室之前已經發表,重組的OmpA蛋白L1-3,L2-3,和L2-4片段能保護小鼠避免因腦內大腸桿菌感染而造的成死亡。在本研究中,我們將對重組的OmpA蛋白片段在小鼠腦內大腸桿菌感染中的保護機轉進行探討。我們的研究結果顯示具OmpA蛋白的大腸桿菌菌株RS218會誘導表達細胞因子,其中IL-17具有刺激細胞產生促發炎細胞因子的能力。而在重組OmpA蛋白片段L1-3,L2-3,L2-4和L3的存在下,多效性抗發炎因子IL-10顯著地提升,抑制發炎反應;提高了受試老鼠的存活率,且受試老鼠的腦內殘餘細菌量降低。本研究證明特定的重組OmpA蛋白片段具有避免受試小鼠感染大腸桿菌而死亡的保護能力,且與細菌的清除有關。
