Abstract
椎間盤(Intervertebral disc, IVD)退化是下背痛的主因之ㄧ,約有九成以上的成人會發生。IVD沒有血管通過,是一個封閉的軟骨組織,必須靠上下椎體滲透養分,所以當受到損傷時,不易自我修復或再生。當造成退化及慢性疼痛時,總花費許多醫療支出。本實驗利用生長因子療法與幹細胞療法促進IVD軟骨的新生或減緩退化情形。首先分離出豬的間葉幹細胞(Mesenchymal stem cells, MSCs)與血小板富含生長因子(Platelet-rich plasma, PRP)後,檢測PRP指標濃度:TGF-b1。先利用不同濃度的PRP指標濃度:TGF-b1,分別是100pg/ml、200 pg/ml、750 pg/ml、1ng/ml、2ng/ml及1%FBS培養豬的NP cells及MSCs-GFP,觀察增生情況,結果以TGF-b1:1ng/ml的PRP為最佳濃度。之後並利用TGF-b1:1ng/ml的PRP,分別與豬的NP cells及MSCs-GFP培養九天,觀察分化情況,在第二天與第九天觀察軟骨基因只有NP cells有增強。在動物體內實驗:以X光照影模式定位出IVD後,使用木瓜酵素(Chymopapain)建立豬的IVD退化模式一週後,再分組注射PRP、MSCs-GFP。期間測量IVD高度(Disc height index, DHI),發現治療組DHI接近Normal組。兩個月後犧牲豬隻,取出NP組織,觀察軟骨基因:Aggrecan與Collagen II的變化,發現不治療組(Chymopapain)並無發現Aggrecan與Collagen II,而治療組仍有Aggrecan與Collagen II表現。組織切片染色:Alcian blue stain、Hematoxylin & Eosin stain及免疫組織學染色:Collagen II,發現治療組的細胞型態、細胞間質醣蛋白與Collagen II的沉積都接近Normal組。綜合以上結果發現使用PRP或MSCs-GFP治療都比Chymopapain組,有減緩退化的成效。
