Abstract
「服藥配合度因素量表」(Medication Adherence Reasons Scale; MAR-Scale)為一新發展之測量服藥配合度量表,此量表曾於患有高血脂以及氣喘的美國族群進行測試,其結果顯示此量表於實務上有潛力可用來測量病患服藥不配合之因素,以提供更精確的衛教與介入。本研究之目的在於建構適合高血壓病患使用之「服藥配合度因素」之中文化量表,測試此量表之文化適合度與心理計量特性,並且研究會響高血壓病患服藥配合度之相關因子。
本研究以翻譯與再議程序方法(forward-backward translation)完成中文版量表,並且根據過去臺灣文獻指出會影響高血壓病患服藥配合度之因素,加入之新題項進入量表。量表初稿完成後由5名專家與10名患有高血壓知病患針對量表之內容進行效度測試,針對專家以及民眾意見修改問卷後始定稿。最終版問卷之測試族群為患有高血壓並且服用至少一種高血壓藥物之病患,問卷發放地點為一間區域醫院與八間社區藥局。研究分析以項目分析、因素分析、內部一致性分析以及效標關聯分析測量問卷之信效度。
問卷發放時間為2016年2月至5月,問卷回收574份而有效問卷為538份。因素分析結果萃取出6因素,而此6因素可解釋56%之總變異量。信度分析結果顯示柯能畢曲係數 (Cronbach’s α) 介於0.649~0.852,而修正的項目總相關 (Item-Total Correlation) 介於0.362~0.719。效標關聯分析則以相關係數來呈現量表與效標測量的關係,服藥配合度因素量表與視覺類比量表 (VAS) 之相關性為0.525,與綜合性問題1 (過去7天有幾天可依照指示服用血壓藥) 之相關性為0.611,與綜合性問題2 (過去30天可依照指示服藥的頻率) 之相關性為0.615。臺灣版服藥配合度因素量表可辨別出62%服藥不配合的病患,而其中最常見會影響病患配合度之因素包含忘記、擔心藥物之副作用或不良效果、以及根據身體狀況調整用藥。最少見會影響病患服藥配合度之因素為服用中草藥、民俗療法以及藥物價格。
本研究為臺灣第一個翻譯「服藥配合度因素量表」並且確立信效度的研究。本研究之優點亦包含樣本數大、心理計量特性表現良好,並且提供更多服藥不配合原因於量表內,可於臨床執業或介入性研究中使用。未來此量表亦須於其他族群或疾病進行測試,以增進外推性。
