Abstract
自體免疫性胰腺炎(AIP)是IgG4相關性疾病的一種表現,特徵為血清中的免疫球蛋白IgG4濃度升高,然而使用血清IgG4濃度的升高來診斷AIP缺乏足夠的敏感性和特異性,因為在胰臟導管腺癌(PDAC)的患者中也會發生此種現象,同時有些AIP患者的胰臟中會表現假性腫瘤病變的症狀,導致在鑑別AIP和PDAC之間產生的困難。本研究以超高解析液相層析質譜儀 (UPLC-MS/MS) 作為分析工具評估AIP和PDAC患者之間的免疫球蛋白醣基化 (glycosylation) 特徵,結合親和純化IgG4的前處理方法以及穩定同位素標記(stable isotope labeled) 的內部標準品;對IgG4-Fc醣肽(glycopeptide)進行相對定量。除此之外也針對IgG4-Fab區域(Fragment antigen-binding region)的醣基化模式進行比較,由於Fab區域中的胺基酸序列是屬於無法預測的可變動性序列,通過可切割IgG4的酵素FabRICATOR®分離其Fab和Fc區域,釋放位於IgG4 Fab區域的聚醣(Glycan)並利用過甲基化(permethylation)穩定聚醣結構、分離不同的異構體以及提高電離效率。將優化完成的工作流程應用於45名受試者並比較健康受試者、PDAC患者以及AIP患者的IgG4醣基化模式,結果呈現雙半乳醣基化(digalactosylation)、岩藻醣基化(fucosylation)和唾液酸化(sialylation)在AIP患者的 IgG4 Fc region中高於 PDAC患者,也發現IgG4 Fab 醣基化的趨勢與Fc不同,PDAC 患者的岩藻醣基化比 AIP 患者更高。本篇研究開發的工作流程可以實現以UPLC-MS/MS 分析IgG4 Fab 和 Fc 區域醣基化特徵,並應用於疾病生物標誌探勘以及協助臨床診斷。
