Abstract
本研究旨在探討認知與腳踏車訓練之雙重任務介入對社區可逆性認知衰弱高齡長者之認知功能及身體功能改善成效:採用單盲隨機控制試驗設計,以北部某十五家社區關懷據點之可逆型認知衰弱老年人為研究對象,經由電腦亂數隨機分派成實驗組七間(33人),對照組八間(35人);實驗組接受為期八週、每週一次、每次55分鐘之固定式腳踏車訓練結合互動式認知訓練;對照組則是接受等時數之認知訓練;資料收集時間為介入前及介入後,並使用蒙特利爾認知評估量表(MoCA)、椅子起立測試(CST)及起立走動測試(TUG)評估認知功能與身體功能表現。研究結果在認知功能顯示,經八週認知與腳踏車訓練之雙重任務介入後實驗組MoCA 總分由前測26.42分,介入後上升至27.64分(p=0.005);視覺空間/執行次量表由前測4.09分,介入後上升至4.48分(p=0.035) ,介入前後有顯著差異,而在命名、專注、抽象概念、延遲記憶以及定向功能等次量表則無顯著成效;對照組方面,MoCA 總分由前測25.80分,介入後上升至26.80分(p=0.020),介入前、後有顯著差異,而在視覺空間/執行、命名、專注、語言能力、抽象概念、延遲記憶、定向功能等次量表組內皆未達統計學上顯著差異,採用獨立樣本t檢定分析結果顯示,二組介入前、後之認知功能(MOCA總分及次量表)差分則無顯著差異。
研究結果在身體功能顯示,在控制維生素D與骨科疾病後,分析起立走動測試結果顯示,實驗組比對照組在介入後起立走動測試秒數會減少1.04秒(p=0.025);椅子起立測試結果顯示,實驗組比對照組在介入後椅子起立測試秒數減少1.37秒(p = 0.053),顯示介入效果未達顯著。
本研究驗證認知與腳踏車訓練之雙重任務介入有助改善社區可逆性認知衰弱高齡長者之身體功能,結合認知與腳踏車訓練之雙重任務介入可作為社區可逆性認知衰弱高齡長者身體功能之介入模式,以改善社區長者身體功能。
