Abstract
背景:目前有越來越多的流行病學證據說明可改變的生活方式因子對頭頸癌風險的影響,但很少有研究在建立頭頸癌風險預測模型或工具時,能按性別和腫瘤部位分別探討,或是將遺傳易感受性納入風險預測模型中。因此本研究的目的是建立不同性別的頭頸癌風險預測模型、分別建立男性頭頸癌中不同腫瘤部位(口腔、口咽、下咽)的風險預測模型及嘗試加入基因多形性以觀察口腔癌風險預測模型的改進程度。
方法:本研究是病例對照研究設計,納入長庚紀念醫院2961例頭頸癌的病例組和台灣人體生物資料庫11462例的健康對照組;遺傳因素將挑選基於全基因組關聯研究中發現與頭頸癌顯著相關的單核苷酸多形性 (SNP)來代表。使用結構式問卷收集研究對象可改變的和不可改變的危險因子,病例組臨床資料聯結自醫療紀錄。利用多變項羅吉斯回歸和列線圖(Nomogram)建立頭頸癌的風險預測模型,也利用自助抽樣法(bootstrapping)對預測模型進行內部驗證。
結果:多變項羅吉斯回歸分析中顯示性別、年齡、教育程度、父親氏族、喝酒、抽菸、二手菸、嚼檳榔、喝茶、喝咖啡和身體質量指數是所有頭頸癌的預測因子;年齡、教育程度、喝酒、抽菸、二手菸、喝咖啡和身體質量指數是兩性共同的頭頸癌的預測因子;而父親氏族、嚼檳榔和喝茶是男性頭頸癌特有的危險因子。至於男性腫瘤部位分層的預測模式中,危險因子皆和頭頸癌相同,唯獨喝茶與口腔癌有關,喝咖啡與口腔癌和下咽癌有關。此外,發現喝酒、抽菸和嚼檳榔三個因子在男性不同腫瘤部位的風險皆不相同;喝酒對下咽的風險最大[勝算比(odds ratio, OR)=14.32]、口咽次之(OR=9.67)、口腔最小(OR=2.98);抽菸對下咽的風險最大(OR=6.94)、口咽次之(OR=3.35)、口腔最小(OR=2.39);嚼檳榔對口腔的風險最大(OR=6.34)、下咽次之(OR=4.74)、口咽最小(OR=3.68)。以上所有預測模型的C-index在0.94~0.98之間皆顯示高準確度且利用校準圖也顯示有好的校準能力。此外,將多個顯著風險基因放入男性的口腔癌預測模型中與原模型相比略有邊緣顯著改進 (P value=0.057)。
結論:本研究開發了多個頭頸癌風險預測模型;不同性別的頭頸癌危險因子會有差異,口腔、口咽、下咽的癌症雖然具有相同的危險因子但有強弱上的差異,且在男性口腔預測模型中加入遺傳因子後也可以稍微改善模型的預測能力。因此研究中的風險預測模型可以用於識別不同性別和不同腫瘤部位的高風險個體,進一步運用於健康促進計畫。
