Abstract
背景:健保雲端藥歷系統為臺灣衛生福利部中央健康保險署於民國一○三年所建置之病人用藥即時查詢系統,目的為整合病人於不同健保特約醫事機構之用藥史。本研究自民國一○四年七月一日至九月三十一日以全國性調查問卷探討健保雲端藥歷系統對臺灣醫師及藥師使用之影響與可行性。
方法:本研究經由文獻回顧,並參考專家會議所發現之議題,編制問卷,以了解醫師及藥師使用健保雲端藥歷系統之滿意度、態度和使用意願。
結果:本研究共收回有效藥師問卷895份及有效醫師問卷105份,並進行分析。研究結果顯示,醫師及藥師對執行藥物連貫性評估(medication reconciliation)均有高的接受度,並與使用健保雲端藥歷系統之意願有顯著的正向相關性(β=0.192, p<0.001);但醫師及藥師對健保雲端藥歷系統之系統品質滿意度則有待改善。此外,大多數醫師及藥師皆同意,比起由各醫療院所請病人簽署同意書,由健保署來執行同意書之簽署是較為合理的(4.02±1.19vs. 3.49±1.40, p<0.01)。
結論:健保雲端藥歷系統有其臨床上使用效益,並提供醫療人員一個方便的整合性用藥資訊,協助用藥連貫性評估之執行。然而,目前仍須改善健保雲端藥歷系統之系統品質,並提升醫療人員對用藥連貫性評估之認知,使健保雲端藥歷系統能發揮其最大之效益。相關政策、推廣活動以及標準作業程序,包含病人資料隱私之保障,皆須完整的建立,才能使此符合用藥連貫性評估(medication reconciliation)精神之雲端藥歷系統生根於台灣之醫療體系中。
