Abstract
促紅血球生成素產生的肝細胞受体與肝配蛋白(Ephs/ephrins)的交互作用會廣泛地影響及調控生物體功能且關係到人類癌症的發病機制。促紅血球生成素產生的肝細胞受體A2型 (EphA2) 屬於一種酪氨酸激酶,可以與 ephrins (例如:ephrin-A1) 結合來引發細胞間雙向的信號傳導,EphA2與ephrin-A1的結合會導致Ras-MAPK活性的抑制進而抑制腫瘤生長,然而研究也證明了EphA2 的過度表現可以驅動所謂配體非依賴性信號並誘導癌化的發生,因此,EphA2對腫瘤的長可具有負面或正面影響。在腫瘤發生過程中,EphA2 與 ephrin-A1 間的正常相互作用會被影響並導致 EphA2的過度表現而走向癌化。胰臟癌是一種非常惡性的腫瘤,其5年存活率僅僅只有9%,EphA2的過度表現在胰臟癌的診斷以及治療中被認定為值得注意的腫瘤標的。在本研究中,我們利用噬菌體展示技術,針對EphA2分子結構上所呈現出的活性部位,分離出可特異性結合EphA2的抗體,除了具有優異的結合力外也證實可以顯著地抑制胰臟癌細胞的生長與遷移,以及造成 EphA2 分子的降解,訊息傳遞訊號pAKT、pERK、pFAK和pSTAT3的減弱進而去抑制胰臟癌細胞的增生和爬行能力。人類化抗體IgG hSD5的結合力為2.06 nM,在BxPc-3和Mia PaCa-2胰臟癌腫瘤異體移植小鼠模型中有效的抑制腫瘤生長,其腫瘤抑制率分別為54.3%和67.5%,當IgG hSD5抗體與Gemcitabine合併使用時,發現可以有效的促進腫瘤抑制的效果。胰臟癌是一種高度侵略性的癌症並且癒後不佳,需要開發新的治療策略。我們先前的結果已經證實,IgG hSD5抗體有效抑制胰臟癌腫瘤生長,並且這個抗體誘導EphA2的降解同時促進抗體的內吞作用,深入的分析hSD5 Fab晶體結構,發現hSD5上的CDRH3主要與EphA2活化位交互作用。我們開發ㄧ種針對胰臟癌並且以EphA2為目標的抗體藥物複合體hSD5-vedotin,此抗體藥物複合體針對腫瘤特異性抗原EphA2,觸發內吞作用並且釋放MMAE分子以達到殺死癌細胞的效果。已證實hSD5-vedotin進入細胞後,展現顯著的腫瘤抑制反應,即使在較低濃度下也能抑制腫瘤生長並且促進細胞凋亡。在胰臟癌異體移植小鼠模型中,hSD5-vedotin完全展現了抑制腫瘤生長的潛力,值得注意的是,在復發的胰臟癌腫瘤中,發現潛在的免疫抗性。我們的結果證實了進一步開發hSD5-vedotin的可能性,預計這將會胰臟癌與其他EphA2相關癌症產生更廣泛和更有效的治療影響。
