Abstract
骨質疏鬆症隨著全球人口年齡老化,在全球的盛行率已逐年上升,且成為常見且後續花費極高的疾病。造成骨質疏鬆以及其併發骨折的原因,除了年齡、性別外,也包括眾多遺傳因素及環境因素。近來,許多團隊利用全基因體掃描研究發現許多位點的基因分型與骨質疏鬆症之易感性有相關,但其研究對象主要為高加索人種,缺乏台灣族群的資料。蝕骨細胞進行骨重吸收、成骨細胞進行骨形成,兩者互相制衡使骨骼處於動態代謝平衡。當蝕骨細胞活性比成骨細胞高時,將會減少骨質密度進而導致骨質疏鬆症。RANKL-OPG為蝕骨細胞與成骨細胞間互相溝通的訊息傳遞路徑。蝕骨細胞主要經由T細胞的訊息刺激而成熟,而鈣池調控鈣離子通道為T細胞中主要調控鈣離子訊號的來源。為了解上述重要機轉中基因群對於骨質疏鬆症的易感性之影響,我們將在台灣族群中進行基因關聯性探討。此研究由台北醫學大學萬芳醫院神經外科診間收納共1150位病人,其中78%為女性。本研究經由基因檢測之結果與所收集的不同部位骨質密度經分析及統計後,得到基因多型性與骨質疏鬆症間的關聯性。在女性族群中,STIM1 rs3750996與脊椎與股骨頸骨密度皆顯著相關。ITPKC中的許多位點,包括rs7251246、 rs10420685及rs2290692皆在男性族群中與骨密度有顯著相關性。而TNFRSF11B rs2062375 在女性股骨頸密度的顯著性更通過巴式校正。根據我們的結果,這些基因的單ㄧ核苷酸多型性皆可能對於骨質疏鬆症的疾病易感性有影響,而本研究也如同先前文獻有觀察到位置特異性。日後期望可將所得的結果應用在預防醫學,以減少台灣族群骨質疏鬆的發生率,及幫助臨床醫師在治療選擇能有更精準的決策。
