Abstract
感染性心內膜炎(IE)是一種發生於心血管系統的高致死率感染性疾病。感染性心內膜炎的臨床治療需要長時間使用高劑量的抗生素,且有高機率再復發。戈登鏈球菌是一類口腔共生菌,不僅會導致齲齒,也是感染性心內膜炎的伺機性感染病原菌。順著傷口進入血液循環的細菌會附著在受損的心臟瓣膜上形成贅生物。贅生物是由細菌生物膜、血小板、纖維蛋白和白血球胞外網狀結構(NETs)等多種宿主因子共同聚集形成的複合物,其多層結構能阻擋抗生素與細菌接觸。我們實驗室過去的研究表明血小板引起的細菌生物膜增生是促進贅生物形成的關鍵之一。另有資料顯示三個戈登鏈球菌細胞壁相關蛋白—Hsa、PadA和SspAB,可能參與了戈登鏈球菌和血小板之間的結合及活化。細菌雙分子調控系統(TCSs)是細菌中廣泛存在的訊息傳遞系統,分為histidine kinase (HK)及response regulator (RR)兩部分,能感應外界訊息並透過一系列的訊息傳遞途徑來調控細菌基因轉錄。我們之前的實驗發現,戈登鏈球菌雙分子調控系統之中的RR4、RR12及RR13會參與調控細菌在血小板中形成生物膜的能力。在感染性心內膜炎的老鼠實驗模型內,被rr4、rr12和rr13剔除菌株感染的老鼠心臟瓣膜上產生的贅生物體積明顯減少。因此,本研究目的為探討戈登鏈球菌TCS中的RR4、RR12及RR13在血小板引起的生物膜形成中扮演的角色與機制,著重於它們是否會通過調控Hsa、PadA和SspAB等蛋白的表現來影響生物膜形成。首先,我們建立了hsa、padA和sspAB剔除菌株,確認hsa、padA和sspAB剔除菌株的生物膜形成會顯著減少。接著使用RT-PCR技術放大從rr4、rr12和rr13剔除菌株萃取出的RNA,發現在rr4、rr12和rr13剔除菌株中,Hsa、PadA和SspAB的RNA轉錄水平比起WT有所降低。另外也對rr4、rr12和rr13剔除菌株做了血小板凝集測試,發現比起WT,rr4、rr12和rr13剔除菌株引起血小板凝集的時間顯著延長。為了追蹤戈登鏈球菌中的Hsa、PadA和SspAB蛋白表現,我先透過基因轉殖的方式使E. coli表現重組蛋白,再把重組蛋白注射進大鼠體內以製造抗體血清。另一方面,我建立了rr4回補菌株以驗證RR4的功能,經PCR確認後,測量了生長曲線。同時也建立了RR4重組蛋白的表現菌株來製造抗體,以便後續對RR4蛋白表現的確認和追蹤。目前為止的研究結果顯示,在有血小板的環境下,RR4、RR12及RR13可能是透過影響Hsa、PadA和SspAB的轉錄來調控生物膜形成以及血小板凝集能力,從而促進贅生物形成,導致感染性心內膜炎的病程發展。
