Abstract
子宮與尿道之間的交互作用是婦科與泌尿系統功能障礙合併症的基礎。在
糖尿病患者的早期階段,高血糖引發的多尿而導致膀胱肥厚並伴隨神經性改
變。然而,糖尿病患者中子宮-尿道交互作用 (uterus-urethra crosstalk, UUC) 是否
為糖尿病之病程所敏化仍然不清楚。本研究探討糖尿病是否通過敏化 UUC 促進
由子宮損傷引起的繼發性泌尿疼痛。因此,將 SD 大鼠分為對照組 (NAV 組) 和
糖尿病組 (DM 組),後者在間隔一週的間隔內腹腔注射兩劑鏈脲佐菌素
(streptozotocin, STZ)。在最後一次 STZ 注射後的第 4 週,透過測量空腹血糖值
是否高於 250 mg/dl 來確認糖尿病是否成功。藉由記錄急性芥子油 (mustard oil,
MO) 灌 注 子 宮 後 的 外 尿 道 括 約 肌 肌 電 圖 (external urethral sphincter
electromyogram, EUSE) 活性來評估 UUC ,在此紀錄期間使用導管倒出尿液保
持膀胱排空。已知 SGK1 在發炎性與神經病變性疼痛的形成與維持過程中,扮
演關鍵的調控角色,我們透過脊隨注射 GSK-650394 抑制 SGK1 作為糖尿病抑
制組 (DM+GSK 組)。
結果顯示,在 NAV 組動物中,子宮的 MO 急性刺激隨著劑量逐步提升增
加測試刺激 (test stimulation, TS) 所誘發 EUSE 的動作電位數量,從而敏化骨盆
尿道反射 (pelvic-urethra reflex, PUR) ; 而注入玉米油 (corn oil, CO ; the vehicle
solution of MO) 則幾乎沒有影響。與 NAV 組相比,DM 組中給予 MO 急性刺激
後 PUR 活性顯著增加。 DM+GSK 組則能有效抑制 DM 大鼠子宮-尿道交互作用
的敏感化。綜上所述,證實急性子宮刺激會增加 PUR 活性,而透過 SGK1 抑制
劑能有效降低跨臟器敏化。
