Abstract
根據我國衛生福利部2017年統計:2016年慢性腎衰竭耗用新臺幣425億元的支出,為重大傷病門診負擔之冠。心血管疾病(cardiovascular disease, CVD)為造成血液透析(Hemodialysis, HD)患者住院與死亡的主要原因。CVD危險因子中的血脂異常、糖尿病、營養不良、慢性發炎等(Sarnak et al., 2003),可藉由飲食來改善甚至達到預防效果(Garcia-Layna et al., 2017)。飲食評量指標分數越高,疾病風險越低(McCullough & Willett, 2006)。本研究室至2010年起以HD患者飲食建議與臺灣百年版每日飲食指南,發展出血液透析飲食評量指標-2014 (hemodialysis eating index, HDEI-2014),並以文獻回顧將其修改為mHDEI-2017 (modified hemodialysis eating index-2017) ,本次研究目的欲探討mHDEI-2017總分與住院和死亡的相關性。材料與方法:前瞻性追蹤研究。試驗通過人體試驗倫理委員會審查(TMU-JIRB: 201302024; CGH-IRB: CGH-OP104001; TCH-IRB: 04-M11-090),分別在臺灣地區7間醫療院所的洗腎中心收案,共計招募390位HD患者,不計入1位因住院天數極端者,20位轉院者,1位動向不明者,368位納入分析,其中47位為死亡者,7位為移植者,於基線期以病歷回顧收集血液生化數值、3天飲食資料,以mHDEI-2017 HD患者飲食資料;追蹤期以病歷回顧收集血液生化值、住院及死亡資料,以Cox regression分析hazard ratio。結果:mHDEI-2017總分高分組不論在有無校正性別與年齡的模式下,均有顯著較低的死亡危險比,並有較高的存活率。結論: mHDEI-2017分數越高代表飲食攝取量與飲食品質越佳,有較低的全因死亡風險及較佳的存活率,亦即mHDEI-2017分數越高預後越好。
