Abstract
背景:惡性肋膜積水是癌症患者常見的併發症之一,象徵疾病進展及預後不佳,常伴隨嚴重呼吸困難、胸痛、咳嗽與疲倦等症狀,對患者生活品質造成重大影響。然而,目前國內外針對惡性肋膜積水患者生活品質與照護需求之整體現況與影響因素之研究仍有限,需進一步探討,以提供更具整合性之照護指引。
目的:了解惡性肋膜積水患者之生活品質、照護需求的現況以及影響生活品質的相關因素。
方法:本研究採用混合研究法,以立意取樣方式,進行量性與質性資料收集。量性部分採用歐洲癌症治療與研究組織癌症生活品質核心問卷第三版臺灣中文版、中文版支持性照護需求調查簡表評估生活品質與照護需求現況,並以線性迴歸分析生活品質影響因素;質性部分則以半結構式訪談並以主題分析法深入探索,透過質性與量性資料的整合,使研究結果相互印證,形成互補,增強研究整體的深度與完整性。
結果:本研究納入32位惡性肋膜積水患者。量性結果顯示,生活品質受身體功能受限與呼吸困難、疲倦、疼痛等症狀影響,並與生理及日常生活需求、mMRC以及KPS呈顯著相關。質性分析呼應此發現,指出症狀嚴重削弱患者自理能力,需倚賴照顧者協助。28.98%患者具健康資訊中高度需求,質性主題亦反映資訊不足與就醫流程繁複導致症狀無法及時獲得處理。心理照護為第二高需求構面,與生活品質顯著相關,質性資料顯示患者常因病情反覆與死亡焦慮影響生活適應。此外,雖量化結果顯示主要照顧者與婚姻狀態對生活品質影響不一致,但質性資料呈現照護模式的複雜性:自我照護者在症狀加劇時需在日常生活中妥協,而部分未婚者因無家庭牽掛或有手足分擔照護責任,反而能較穩定因應病情。此發現突顯臨床評估應關注患者實際照護資源與支持網絡,而非僅依人口學特徵判斷其照護需求。
結論:透過質量並重的混合方法研究發現,惡性肋膜積水患者的生活品質主要受症狀困擾與功能受限影響,並存在顯著的健康資訊與心理支持需求。研究揭示照護模式因人而異的複雜性,強調臨床照護應以患者實際支持資源為評估基準,制定個別化的症狀處置策略與心理支持介入。
