Abstract
人口老化是全球性的趨勢,在台灣也不例外,根據內政部統計至102年底,老年人口已達到11.53%,且台灣長期照護機構至年底已達到1035家,使用率更是達到75.4%。居住在機構中的老年人有許多的健康問題,例如多種慢性病、多重用藥、跌倒、營養不良等等,而機構中的日常活動包含了生活照顧、三餐供應、疾病照護等等,且整天活動中又以用餐最為重大,也最被老人及家屬所關心,最能直接觀察就是個案的營養狀況了,若是營養狀況不佳會衍生許多健康問題,所以做好營養評估對於老年人的健康是重要的,故本篇研究針對三種營養評估工具(迷你營養評估表、身體質量指數及基本資料庫)進行探討,最後並討論其適用性。
本研究採橫斷式進行探討基本資料庫台灣版於評估長期照護機構長者營養狀況之適用性。個案來源採立意取樣(purposive sampling)篩選符合條件之住民進行資料收集,研究場所為北部地區立案之長期照顧機構,收案條件:(1)年滿65歲或65歲以上者。(2)在該長期照護機構居住滿3個月以上。(3)同意參與研究者,並簽屬同意書。研究工具為自擬住民基本屬性資料表、迷你營養評估表(mini nutritional assessment, MNA)、基本資料庫(minimum data set-nursing home, MDS-NH)、身體質量指數(body mass index, BMI),採用PASW/18.0 PC 統計軟體進行分析,以次數分配、百分比、平均值標準差、最大值、最小值、卡方檢定、One-way ANOVA、邏輯斯迴歸、線性迴歸、次序迴歸進行分析。
研究結果顯示,收案對象為131位,平均年齡為80.28±7.98歲,在迷你營養評估量表方面,有36.6%為營養狀況正常、30.5%為具營養不良風險及32.8為營養不良;在身體質量指數方面,有20.6%的住民BMI小於18.5kg/m2、52.7%的住民BMI介於18.5-24 kg/m2之間及26.7%的住民大於24 kg/m2;基本資料庫-營養評估部分,有71%的住民營養狀況正常,另外有29%的住民有營養狀況問題。在鑑別力的部分,若以MNA為基準,MDS之敏感性為35.7%,特異性為82.9%,準確度為52.6%;以BMI為基準時,其敏感性為44.4%,特異性為75%,準確度為68.7%。
就以上結果可得知,MDS在MNA與BMI上敏感性較低但特異性皆較高,故若是要使用MDS營養部分作為篩檢工具時,建議可使用在MNA或BMI後的重複測量,以排除掉真正無營養狀況之族群,進而找出遺漏掉之可能有營養問題的族群。
