Abstract
背景:思覺失調症隨著時間的推移,對個人和社會均造成了顯著的疾病負擔。為了增強對思覺失調症的管理並減少相關的自殺問題,制定有效的策略和法規至為重要。我國衛生福利部於2010年實施思覺失調症論質計酬方案[the Pay-for-Performance(P4P) Program for Schizophrenia],然而國內學者對於該論質計酬方案之成效進行評析的研究相對甚少,且僅聚焦於患者的醫療照護利用之分析。因此,本研究的目的旨在探討論質計酬方案對思覺失調症患者自殺風險的影響,以補強相關的研究缺口。

方法:本研究採用全民健康保險學術研究資料庫進行次級資料分析,並採用回顧性研究設計。研究對象為2009年至2017年間,符合我國思覺失調症論質計酬方案固定就醫組別標準之患者(P4P組),以及未符合該標準之患者(未參與P4P組)。使用患者特質、醫師特質、醫院特質以及指標年(Index Year)計算傾向性分數,並以1名P4P組個案:4名未參與P4P組個案的比例配對。統計分析方法係以卡方檢定、Kaplan-Meier模型與Cox比例風險模型進行分析,以檢定研究假設。

結果:本研究共納入2,118名參與P4P方案與6,299名未參與P4P方案的患者。統計分析結果顯示,參與P4P方案的患者在一年及三年內自殺風險分別較未參與者降低32% (adjusted HR=0.68, 95% CI=0.58-0.79)與31% (adjusted HR=0.69, 95% CI=0.60-0.81)。此外,男性、高齡、共病症指數較高、所屬健保分局位於臺灣東部以及接受由男性醫師照護的患者,其自殺風險皆顯著較高。而接受年資10年以上醫師照護的患者其自殺風險則顯著較低。至於醫院所有權、評鑑等級及是否為教學醫院,則與自殺風險無明顯的關聯性。

討論:本研究證實P4P方案對降低思覺失調症患者自殺風險具臨床價值,並識別出高風險群體。建議主管機關擴大P4P方案推廣,在臨床上加強高風險患者的識別與管理,並重視醫師臨床經驗與跨性別照護訓練,以提升整體醫療照護品質。
