Abstract
背景:
中央肥胖是代謝與心血管健康的關鍵決定因子,與收縮壓(SBP)有顯著關聯。雖然全基因組關聯研究(GWAS)已鑑定出與肥胖相關的遺傳位點,但大多數研究主要以身體質量指數(BMI)作為整體肥胖的指標,且多集中於歐洲族群。針對亞洲女性中央肥胖的遺傳基礎及其對SBP的影響,仍有待深入探討。

研究目的:
本研究旨在:(1) 量化中央肥胖對女性SBP的影響,(2) 描述肥胖指標的遺傳架構,(3) 評估中央肥胖的遺傳傾向對SBP的影響,並探索其潛在的生物學機制。

研究方法:
本研究使用台灣生物資料庫的資料,納入具有完整表型與基因型資料的女性受試者,包含臨床、體位與遺傳資訊。中央肥胖以腰圍(WC)、腰臀比(WHR)與腰高比(WHtR)進行評估。進行GWAS以鑑定與中央肥胖相關的遺傳變異,並建構多基因分數(PGS)以評估遺傳傾向。利用線性混合模型評估中央肥胖與SBP之間的關係,調整自然、生活型態與代謝因子以及BMI。進一步以廣義加性模型(GAM)分析總體與中央肥胖PGS與代謝因子在SBP中的交互作用。並透過交互作用分析、因果中介分析與多基因途徑分析探討中央肥胖遺傳傾向對女性SBP的影響。

研究結果:
中央肥胖指標與女性SBP呈顯著相關,且獨立於自然與生活型態因子,其中以WHtR的影響力最強。然而,在進一步調整代謝因子與BMI後,這些關聯性有所減弱。GWAS分析發現多個與BMI、WC、WHR及WHtR相關的顯著遺傳位點,其中BMI、WC與WHtR具有重疊的遺傳架構,而WHR則呈現出不同的遺傳特徵。PGS-WC與SBP呈顯著正相關(ꞵ = 0.02, 95% CI = 0.001–0.039,線性p值 = 0.037;edf = 1.063,F = 4.058,平滑p值 = 0.045),尤其在正常體重與過重女性中較為明顯,但在進一步調整代謝因子後其效果減弱。值得注意的是,三酸甘油脂與高密度脂蛋白膽固醇比值(TG/HDL-C)顯著修飾了PGS-WC與SBP之間的關係。在正常體重(edf = 4.33, F = 11.15, p < 0.0001)、過重(edf = 3.74, F = 13.58, p < 0.0001)及肥胖(edf = 3.17, F = 9.25, p < 0.0001)女性中,TG/HDL-C與PGS-WC間的交互作用均顯著。此外,TG/HDL-C透過腰圍的中介機制,亦顯著影響PGS-WC與SBP之間的關聯:正常體重(ꞵ = 1.38e-03, 95% CI = 7.51e-04–2.25e-03, p = 2.73e-04)、過重(ꞵ = 1.03e-03, 95% CI = 3.69e-04–2.125e-03, p = 1.66e-02)、肥胖(ꞵ = 9.57e-04, 95% CI = 4.51e-04–1.78e-03, p = 3.99e-03)。補充性途徑分析指出,免疫相關活動與細胞訊號傳導途徑在其中扮演關鍵角色,提供了中央肥胖透過發炎與細胞訊號調控影響血壓之生物學機制的見解。

結論:
本研究結果凸顯中央肥胖在SBP調控中的重要性,並指出遺傳上對腰圍的傾向與血壓升高有關,特別是在具有代謝風險因子的女性中更為明顯。觀察到的遺傳與代謝交互作用強調,應整合遺傳風險、中央肥胖與代謝健康,以進行高血壓的預防與管理。
