Abstract
膀胱癌是全球第十大常見癌症,多數患者初次被診斷為非肌肉侵襲型 膀胱癌。其預後良好,但多數病人會在短期內復發,並且進展為肌肉侵襲型膀胱癌,因此對於非肌肉侵襲型膀胱癌的治療仍需開發新的療法來改善。中藥在華人社會已被使用數千年之久,而中藥於癌症治療也持續受到關注。Bufadienolides 為珍貴中藥蟾酥的主要成分,已於臨床用於多種癌症的治療;然而其於膀胱癌中的療效、具體活性成分及藥理機制並未詳細探討。 本研究著重於 Bufadienolides 家族中 Cinobufotalin 對膀胱癌的抗癌活性及分子作用機制探討。實驗結果顯示 Cinobufotalin 能夠有效地抑制管腔型膀胱癌細胞的存活和分裂,對正常膀胱上皮細胞的毒性極小;在 RT112 異種 移植模型中也觀察到顯著的抗腫瘤效果。此外,Cinobufotalin 能夠誘導細 胞凋亡,抑制過氧化物酶體增殖物激活受體 γ(PPARγ)及其下游蛋白的表現,並且減少脂滴(lipid droplets)和脂肪酸的合成。透過代謝體分析也發現 Cinobufotalin 顯著降低 RT112 細胞中的 L-carnitine 和 acylcarnitine 的含量。PPARγ 的過度表現逆轉了 Cinobufotalin 所誘導的細胞凋亡,表示 PPARγ 的下調在 Cinobufotalin 誘導的細胞凋亡中扮演著關鍵的角色。透過免疫共沉澱和 siRNA 抑制基因表現,我們首次確認 Cinobufotalin 在管腔型膀胱癌中是透過 SIAH1/2 所介導的泛素蛋白酶途徑來降解 PPARγ。綜合以上實驗結果,Cinobufotalin 為未來膀胱癌治療相當有潛力的藥物。
