Abstract
目的:探討台灣老年人2008至2016年創傷性腦損傷流行病學之趨勢,以及檢視人口學及疾病特性對創傷性腦損傷趨勢變化之影響。
方法:使用2008至2016年北部2間及南部1間醫院之外傷登錄資料庫,共納入15,222位創傷性腦損傷患者,其中65歲以上之老年人創傷性腦損傷佔4,069位,由資料庫擷取ICD-9-CM、ICD-10診斷、性別、受傷時間、地點、事故原因、昏迷指數、腦部電腦斷層。以描述性統計以及廣義估計方程式(generalized estimating equation)分析65歲以上老年人創傷性腦損傷之比例變化趨勢,以及探討性別、年齡、受傷原因、嚴重度、住院天數對於創傷性腦損傷中老年人之比例在時間趨勢變化之影響。
結果:創傷性腦損傷患者(n = 15,222)以男性居多(61.67%)、嚴重度以輕度居多(71.73%)、受傷原因以車禍居多(61.67%)、其次為跌倒(落)(31.16%)。65歲以上老年人(n = 4,069)發生創傷性腦損傷,亦以男性為主(58.07%)、輕度創傷性腦損傷居多(72.01%)、主要受傷原因則是跌倒(落)(53.32%)。在2008年至2016年間, 65歲以上老年人創傷性腦損傷比例隨時間有增加之趨勢(由16.86%上升至32.7%,p = 0.001)受傷原因與嚴重度影響時間改變之老年人創傷性腦損傷之比例(p<0.05),性別則不影響隨時間改變之老年人創傷性腦損傷之比例(p = 0.85)。
結論:老年人創傷性腦損傷有逐年增加之趨勢,且老年人之受傷原因以跌倒居多,嚴重度以輕度為主,臺灣目前正處於的老年化社會,此研究結果對於老年人創傷性腦損傷之預防與介入措施的發展具有實質影響。
