Abstract
我國罹癌人數逐年增加,且近年醫療科技發展迅速,健保署為照護癌症所投入的醫療費用也隨之增加,讓政府與民眾愈來愈關注癌症治療給付的相關議題。然而當人們預期壽命並沒有被這大筆金額影響時,其費用的價值是令人質疑的。然而,我國尚缺乏分析醫療費用支出(投入)與癌症存活率(產出)的相關研究。據此,本研究比較不同時期罹患乳癌的病人醫療支出與其存活情形,探討醫療費用與存活之間的關係,以作為健保署日後投入醫療資源政策參考之用。
本研究使用全民健康保險研究資料庫進行次級資料分析,以2002-2004年間與2009-2011年間的乳癌病人作為研究對象,探討二組病人存活狀況與醫療費用之差異,進而推估每增加一個生命年所需的費用。本研究利用2005-2008年間的存活情形預測2009-2011年的存活率,並使用政府公開的生命表進行調整。在費用方面,本研究採用phase-of-care方法 及Kaplan-Meier sample average estimator (KMSA)方法計算醫療費用,並使用二部模型(two-part model)進行估計。
2002-2004年間與2009-2011年間罹患乳癌的病人存活率雖有差異,但差異很小,使用Wilcoxon test時會達顯著差異,Log-rank test時則未達到顯著差異。使用KMSA方式估計出的每個生命年平均費用會高於使用phase-of-care方法的估計。在未調整存活率的情況下,追蹤2002-2004年與2009-2011年罹患乳癌的病人四年,每個生命年成本約為146-472萬元;調整存活率後,每個生命年成本高達506-1,740萬元。若使用phase-of-care 方法追蹤11年,估計出的每個生命年成本為$9,740,000元。
本研究調整存活率後,生命年價值會高的不合常理,與以往的文獻相較,相距甚遠,可能有嚴重的偏誤。此外,若以英國進行成本效益的閾值(threshold)來探討本研究結果,本研究估計出的每個生命年成本都沒有達到英國成本效益閾值。
