Abstract
本研究目的為翻譯失智症評估量表,建立一份中文版「Rowland全球失智症評估量表」,並比較其與MMSE及MoCA於篩檢輕度認知障礙或失智症患者之信度、效度、敏感性、特異性與預測值。
研究方法:本研究為橫斷性的描述性研究(cross-sectional descriptive design),採立意取樣,以結構性問卷量表收集資料,總收案人數共計192人,實驗組樣本來自於北部某兩所醫學中心門診65歲(含)以上懷疑有認知功能障礙之老年人142人,對照組為社區健康中心65歲(含)以上認知功能正常之老年人50人。
研究結果:研究結果顯示中文版RUDAS之再測信度為0.90,MMSE為0.86,MoCA為0.82。中文版RUDAS、MMSE、MoCA之施測者間信度Kappa值為0.88、0.88、0.89。中文版RUDAS具有良好的CVI值為0.97。中文版RUDAS、MMSE、MoCA對門診患者與社區健康老人兩族群間認知狀態皆達統計上之顯著差異,顯示皆具有良好建構效度。研究者用中文版RUDAS來檢測輕度認知功能障礙(MCI)與失智症選擇的最佳切分點分別為25分、22分,敏感性為0.85、0.76,特異性為0.78、0.81,AUC:0.82、0.78,陽性預測值為0.98、0.83,陰性預測值為0.91、0.91;MMSE檢測輕度認知功能障礙(MCI)與失智症選擇的最佳切分點分別為27分、24分,敏感性為0.87、0.83,特異性為0.65、0.84,AUC:0.76、0.84,陽性預測值為0.94、0.79,陰性預測值為0.78、0.87;MoCA檢測輕度認知功能障礙(MCI)與失智症選擇的最佳切分點分別為24分、20分,敏感性為0.88、0.79,特異性為0.74、0.80,AUC:0.81、0.81,陽性預測值為0.93、0.74,陰性預測值為0.74、0.87。
Conclusion:本研究發展的中文版Rowland全球失智症評估量表與MMSE、MoCA相同具有良好之信效度、敏感性與特異性,能提供臨床醫護人員快速、確實、敏感的檢測出認知功能障礙,以及早提供適當的臨床處置。
