Abstract
骨關節炎(OA)是一種致殘性疾病,其特徵為慢性發炎、氧化壓力、缺氧、血管生成驅動的疼痛以及軟骨降解,因此在治療上面臨重大挑戰。在本研究中,我們提出了一種經皮水凝膠(Pluronic F127/甲基纖維素)包封平台,旨在透過多功能組分的協同整合來應對OA的多方面病理。本奈米醫學-水凝膠系統包含血小板來源的細胞外囊泡(pEVs)以發揮抗發炎遞送作用,光治療劑二硫化鉬(MoS2)、辣椒素(CAP)和去甲基阿魏酸二聚體(DIF)以實現強效抗發炎效果,以及氫鍵有機框架(HOFs)以增強藥物包封效率和導電性。實驗結果顯示,pEV/MoS2/DIF/CAP NZ-HOF@F127/MC能夠透過促進巨噬細胞由M1型向M2型極化來緩解炎症,減輕氧化應激,抑制血管生成,並透過經皮光治療調控缺氧微環境。體內大鼠OA模型研究表明,該體系在近紅外線(NIR)觸發光治療下,顯著減少了關節炎症,促進了軟骨再生,並改善了運動能力。 pEV/MoS2/DIF/CAP NZ-HOF@F127/MC的多重作用機制,包括精確藥物遞送、可控釋放和按需光響應,凸顯了其在OA管理中的變革潛力。本研究結果為OA提供了一種有前景的治療策略,能夠有效緩解發炎、氧化壓力和關節損傷,並展現出強大的臨床轉化前景。
