Abstract
帕金森氏症是一個嚴重的神經退化性疾病,會造成運動動作障礙,治療帕金森氏症之藥物可能產生精神症狀,特別是對於已接近或步入老年的患者發生精神症狀的風險更高。過去國外研究發現帕金森氏症藥物可能是罹患精神症狀的風險因子,然而在台灣卻少有相關研究。
本研究利用全民健保資料庫2005年百萬抽樣歸人檔為資料來源,進行回溯性追蹤研究與病例交叉研究,分別針對帕金森氏症患者盛行率、帕金森氏症患者用藥情形與帕金森氏症患者之有精神症狀的用藥進行分析,結果顯示2008年罹患帕金森氏症患者,盛行率為0.32%,未滿40歲0.03%,40歲至60歲0.16%,60歲以上盛行率2.00%。帕金森氏症患者最常使用藥物為多巴胺(Dopaminergics,簡稱L-DOPA)類藥物,第二為抗膽鹼能(Anticholinergic agents)類藥物,第三為鹽酸阿曼他(Amantadine derivatives)類藥物。另外研究發現單獨使用抗膽鹼能(Anticholinergic agents校正後勝算比為2.23,信賴區間1.04-4.79)、多巴胺(Dopaminergics (L-DOPA)校正後勝算比為2.63,信賴區間1.48-4.70)、多巴胺促效劑(Dopamine agonists校正後勝算比為2.67,信賴區間1.15-6.22)等三種藥物,皆有較高機率產生精神症狀。有使用帕金森氏症藥物的帕金森氏症患者相較於沒有使用帕金森氏症藥物的帕金森氏症患者在經過潛在干擾因子校正過後,有顯著較高的精神症狀罹患風險,推論帕金森氏症用藥可能為罹患精神症狀風險因子。而年紀越大、共病指數越高者,精神症狀發生率亦越高。
人口老化的台灣,帕金森氏症個案數將會持續增加;相對的,長期醫療資源用於帕金森氏症的照護也會日益增加,本研究結果提供臨床治療帕金森患者的用藥更審慎評估,期能將副作用降至最低,提昇照護品質。
