Abstract
背景:回顧過去文獻發現,雖然目前已有不少強迫症相關研究,但過去的研究主題大多偏向流行病學、生活品質、心理及臨床層面之探討,有關強迫症患者之醫療利用等研究迄今卻少之又少。本研究擬運用台灣全民健康保險研究資料庫,探討強迫症患者與非強迫症患者醫療利用情形是否有顯著差異。

方法:本研究為採用次級資料庫之回溯性研究,資料來源為中央健康保險署所提供的全民健康保險研究資料庫。自2010至2012年之門診中進行樣本篩選,篩選出強迫症患者(ICD-9-CM:300.3)患者,對照組為非強迫症個案。使用t-test分析探討強迫症患者與非強迫症患者在2013年之醫療利用情形是否有顯著差異,醫療利用率包含一年內的門診次數、平均門診費用、住院天數、費用以及總醫療費用。
結果:本研究控制性別、年齡、每月投保金額、地區與都市化程度,在2013年強迫症患者相較於非強迫症患者使用較多的醫療資源。門診次數多出7.513次,門診支出增加新台幣7,652.5元,住院支出增加新台幣8,209元,支出總額增加新台幣3391.1元。
結論:罹患強迫症之族群在總醫療費用偏高,但未達顯著水準。另發現在平均門診次與費用皆有顯著差異,特別是在精神科門診差距幅度最大。
