Abstract
老年性黃斑部病變 (AMD) 是老年人視力喪失的主要原因之一,預計到2040年全球AMD盛行率將增加至2.88億人。隨著3C電子產品的普及,視網膜病變患者數量不但急劇增加,而且趨勢逐漸年輕化。最近幾年的研究已確認,藍光是3C產品導致視網膜疾病的主要原因之一,其中黃斑部病變已成為眼科公共衛生的主要議題,至今幾乎沒有有效的治療藥物。因此開發治療藍光誘導的視網膜病變逐漸受到大眾重視。來自真菌的天然成分具有易於發酵、大量生產、物種多樣性、未開發和新穎性等優點。本研究使用的蟬花和猴頭菇在過去的文獻中顯示出其具抗氧化、神經保護和免疫調節作用。此外,蟬花作為傳統中草藥之一,亦記載其具有明目的功效,因此選擇它為本研究的主要治療藥物之一。另一方面,猴頭菇含有獨特的萜類分子,已被證實對帕金森氏症具有神經保護作用,可以穿越血腦屏障 (BBB) 。兩種藥用真菌皆顯示它們具有治療眼科疾病的潛力。因此在本研究中,將活性氧化物和Nrf2-Keap1-p62途徑作為藍光誘導的AMD標記,並在視網膜色素上皮細胞 (ARPE-19) 和視神經細胞 (661W) 中建立離體細胞疾病模式。在這兩種細胞中觀察到藥用真菌萃取物能夠降低藍光誘導的細胞內生性活性氧化物產生。此外,也發現蟬花萃取物可以通過抗氧化和自噬作用 (autophagy) 保護細胞,而猴頭菇萃取物則顯示出對661W細胞的保護功效。在動物實驗方面,使用5週齡的C57BL/6小鼠連續照射藍光28天,並成功確立小鼠黃斑部病變疾病模型 (AMD disease model) 。通過口服治療探究藥用真菌蟬花菌和猴頭菇萃取物對視網膜的保護效果。透過頻域光學同調斷層掃描攝影技術發現藍光介入使小鼠視網膜出現玻璃膜疣沈積物 (drusen)、脈絡膜與視網膜色素上皮細胞層中產生細胞外沈澱物以及視網膜總厚度下降。視網膜電圖則證實了長期藍光照射會降低視網膜色素上皮細胞和視神經細胞的電生理功能。體外結果顯示,蟬花萃取物能夠有效緩解小鼠視網膜中藍光誘導的功能和結構損傷,而猴頭菇萃取物則獨特顯示出其神經保護效果。總結,根據實驗結果顯示藥用真菌蟬花和猴頭菇萃取物可能具有預防或治療黃斑部病變的潛在成分,特別是藍光引起的視網膜相關損傷。未來需要進一步的研究以充分確立它們的潛在臨床應用效用。
