Abstract
研究背景:頭頸癌因腫瘤位置特殊及治療副作用顯著,常導致進食困難、體重下降與營養不良。手術可能破壞咀嚼與吞嚥功能,放化療則常伴隨黏膜炎、味覺障礙、口乾及噁心等症狀,進一步降低口腔攝食量(Alshadwi et al., 2013)。約36%接受放療者在第1至第2週出現黏膜炎,第5至第6週上升至60%(Murphy et al., 2009),同步化放療更使其發生率高達98%(Yokota et al., 2016)。營養不良者罹患重度黏膜炎的風險為營養良好者的3.2倍,且高比例病人因味覺異常進食量減少、體重減輕(蔡等人,2008)。為因應營養問題,臨床常採由口進食、鼻胃管或胃造口方式提供營養支持,但其適用性與成效仍缺乏一致見解。尤其在臺灣相關研究有限,需更多實證資料以協助臨床制定個別化營養介入策略。
研究目的:比較不同飲食攝取途徑對頭頸癌病人營養與臨床結果的影響,探討由口進食、鼻胃管餵食與胃造口灌食三種飲食攝取途徑對治療期間身體質量指數(Body Mass Index, BMI)、血清白蛋白(Albumin)、血紅素(Hemoglobin, Hb)、白血球(White Blood Cell, WBC)之變化,以及完成治療一年後之生存狀態與腫瘤復發情形。
研究方法:本研究採回溯性研究設計,分析2018年至2022年間於某醫學中心初次診斷為頭頸癌且接受放射線治療之病人資料,依其主要飲食攝取途徑分為三組,進行營養與臨床結果之比較。並利用GraphPad Prism進行統計分析,包含常態性檢定、單因子變異數分析(One-way ANOVA)或Kruskal-Wallis檢定及事後比較等。本研究經臺北醫學大學暨附屬醫院聯合人體研究倫理委員會審查通過。
研究結果:飲食攝取途徑與BMI及Hb變化呈顯著關聯,特別是在放療有加上化療的病人中更為明顯。三組間BMI絕對與相對變化達統計顯著水準(P = 0.002 與 P = 0.001),Hb變化亦呈顯著差異(P < 0.001),WBC相對變化亦具顯著性(P = 0.001)。進一步分析發現,放療有加上化療的病人在BMI與Hb變化上較無化療者有顯著差異(P < 0.0001);其中Hb與WBC相對變化亦與飲食攝取途徑有統計相關(P = 0.0357 及 P = 0.0136)。飲食攝取途徑與治療後一年之臨床結果有關:由口進食者之存活率與無病存在率皆顯著優於鼻胃管與胃造口組。
結論:不同飲食攝取途徑對頭頸癌病人營養與臨床結果具明顯影響。建議臨床應依個別病人情況提供客製化營養介入策略,並於治療前即納入營養評估與預防性規劃,以提升整體照護品質與治療預後。
