Abstract
目的:在許多醫院,針對老年住院病人跌倒風險因子的評估,多是根據以往既有的評估工具而來,且鮮少被驗證。本病例對照研究之目的即在於探查單一藥物使用、多重藥物使用及其他潛在風險因素與台灣五十歲以上急性住院病患發生住院跌倒之相關性。
方法:自2014年4月至2015年3月的1年期間,在台灣某急性照護醫院共有80位病患於住院期間發生跌倒。每一位跌倒病患在配對其跌倒之病房與時間後,以一比五並採用系統性抽樣的方式,於未發生跌倒的病患中選出其對照組。
結果:研究結果顯示,男性的跌倒風險較女性低了百分之六十二。住在雙人與三人病房的病患其跌倒風險分別為單人病房病患的4.20及5.50倍。再者,內科與神經復健科病患其跌倒風險分別為外科住院病患之3.38與5.74倍。身體質量指數為過重的病患,其跌倒風險較指數正常的低了百分之五十九。服用四到十種與大於十種藥物的病患,其跌倒風險分別為服用零到三種藥物病患的3.64與2.46倍。此外,男性病患若服用阿斯匹靈與苯二氮平類藥物,其跌倒風險為服用同種藥物之女性病患的8.62與4.21倍。
結論:在住院的較老病患中,女性患者、住在多床病房者、內科與神經復健科病患、服用多種藥物者、身體質量指數較低者與跌倒風險的增加有顯著的相關,並且需要更小心的監測。再者,男性使用阿斯匹靈與苯二氮平類藥物,其增加的跌倒風險較女性為高,應小心並減少不必要的使用。
