Abstract
研究背景與目的:第二型糖尿病是全球嚴峻的公共衛生挑戰,且已被證實是多種惡性腫瘤的顯著風險因子。鈉-葡萄糖協同轉運蛋白 2 抑制劑(SGLT2i)作為新型降血糖藥物,雖已證實具備心腎保護益處,但其對癌症風險的影響在現有文獻中仍具爭議,隨機對照試驗多顯示為中性結果,而部分觀察性研究則提示潛在的保護效應。本研究旨在利用大規模真實世界數據 (RWE),評估 SGLT2i 使用與 T2DM 患者整體及特定部位癌症發生率之關聯性。
研究方法: 本研究採用回溯性世代研究設計,納入 2014 年至 2018 年間之去識別化電子健康紀錄。研究對象為 18 至 90 歲且無癌症病史之糖尿病患者,分為 SGLT2i 使用組與非 SGLT2i 類藥物對照組。為降低潛在偏誤,研究透過傾向分數配對法針對年齡、性別、共病症、用藥史及實驗室數值等 60 項特徵進行 1:1 配對,最終兩組各納入 103,341人。分析方法採用Kaplan-Meier曲線評估累積發生率,並以 Cox 比例風險模型估算風險比。
研究結果:在平均約7.19年的追蹤期間內,SGLT2i 組的整體癌症發生率顯著低於對照組(HR: 0.852, 95% CI: 0.790–0.919, p<0.001)。在特定癌症分析中,SGLT2i 組對胰臟癌(HR: 0.598)、大腸直腸癌(HR: 0.686)及肺癌(HR: 0.780)有顯著的風險降低效應。而膀胱癌、乳癌、肝癌、腎臟癌及子宮內膜癌,兩組間無顯著統計學差異。
結論與建議: 本研究證實 SGLT2 抑制劑與 T2DM 患者整體癌症風險降低顯著相關,且在胰臟、大腸直腸及肺部等特定癌別具有潛在預防效益。此發現不僅確立了 SGLT2i 在長期使用下的腫瘤安全性,更支持其具備超越血糖控制的多重效益。本研究展示了如何運用跨國醫療大數據克服傳統研究限制,為藥物再定位與臨床決策支援提供了具備高度外部效度的真實世界證據。建議未來臨床實務可將癌症風險評估納入 T2DM 用藥決策,以實現更全面的病患健康管理。
