Abstract
研究背景:老化被視為結構或功能上的退化狀況且身而為人必經的過程,此樣的過程緩慢的演進,且又以認知功能的退化尤其重要。
研究目的:探討認知功能障礙症的肌少症盛行情形並了解非藥物介入對該群體的成效。
研究方法:本研究分成兩階段進行,第一階段透過統合分析確認認知功能障礙症之中年至老年的肌少症盛行率情形;第二階段藉由統合分析鑑定認知與活動訓練對於認知功能障礙症長者之效果量,依循系統性回顧進行文獻檢所,彙總及精要成效指標為:整體認知功能、執行功能、工作記憶、步態、身體活動及憂鬱,並藉由試驗序列分析法,加以驗證效果。
研究結果:第一階段統合分析共計納入55篇文章,結果顯示中年至老年輕度認知功能障礙症者高達30.1%合併肌少症,而37.7%合併有潛在肌少症的狀態,且年齡越大、女性、較低的身體質量指數、居住在機構、認知功能障礙症屬於失智症、不良的行為(抽菸及飲酒)、罹患共病(憂鬱症、糖尿病、心臟病、高血壓及關節炎)皆為顯著調節變項。第二階段統合分析共計納入20篇文章,結果顯示對整體認知功能、執行功能、工作記憶、步態、身體活動及憂鬱皆顯著效益。
結論:此系列研究可得知,高達大約1/3的中年至老年人受其認知障礙症,且合併於肌少症之困擾,故提供針對身體與認知功能的介入干預為其重要。由統合分析結果證實,同步提供認知與活動訓練之非藥物介入干預,對整體認知、執行功能、工作記憶力、步態、身體活動及憂鬱皆有正向影響,經試驗序列分析加以驗證仍需高品質之隨機化臨床試驗證實。
