Abstract
二氫吡啶類(dihydrpyridine)是用於治療各種心血管疾病的鈣通道阻滯劑(calcium channel blocker [CCB])。它們通過阻斷 L 型鈣離子通道來抑制鈣離子通過細胞膜的流動。目前已知在dihydropyridine-induced gingival overgrowth (DIGO) 成纖維母細胞中的膠原蛋白過度增生中,IL-1–NF-κB–androgen receptor pathway扮演關鍵角色。本研究旨在研究於 DIGO 細胞中, NF-κB pathway是經由那一個機制(canonical-或alternative- 通路)主導調控IL-1NF-κBandrogen receptor 級聯反應(cascade)。
本研究選擇來自 DIGO (n = 5) 和健康 (n = 5) 患者的牙齦成纖維細胞,並用 IL-1β、硝苯地平(nifedipine)或兩者刺激。對於每個原代細胞樣品,所有實驗均一式三份進行獨立實驗。結果顯示這兩種藥物通過刺激 IKKα/β 磷酸化來誘發 NF-κB 活性,甚至還引發了 NF-κB、Rela 和 p50 的核轉位 (*p < .05),並增加DIGO 細胞中 Col I 的產生。經由添加蛋白酶體抑製劑(proteasome inhibitors),其中包括硼替佐米(bortezomib)和 MG132兩種藥劑,不但促進了磷酸化 p-IκBα 的積累,並抑制 p50 和 Rela 的細胞質至細胞核的核轉位 (*p < .05),並減少 DIGO 細胞中對 Col I 的合成。
藉由本實驗結果,我們推斷 IKK–IκBα 活化可由 DIGO 細胞中的促炎細胞激素(如IL-1)和 CCB (如硝苯地平) 加成誘導,並且會觸發下游 NF-κB–Col I 合成。Canonical NF-κB 級聯反應是主導IL-1–NF-κB–androgen receptor pathway內部的主要通路,而 加入蛋白酶體抑製劑可干擾 IKK–IκBα–NF-κB–Col I 通路,可策略性地運用到預防或抑制DIGO 的療效。
