Abstract
背景:失眠是最普遍的睡眠障礙,不會危及生命卻影響生活品質甚鉅。zolpidem為全臺灣安眠藥品使用量第一,然而長期使用可能造成安全性、耐受性、依賴性、藥物濫用等問題。病人就醫行為可視同為消費者行為,臺灣以健保藥品給付規定限定醫師開立藥品,但未限制病人就醫選擇及次數,可能因此造成病人以逛醫師行為重覆領用安眠藥品。逛醫師行為會浪費醫療資源,可能因此延誤診治實際上需要接受醫療照護的病人,並可視為藥品濫用的溫床。因此如何降低病人以逛醫師行為重覆領用安眠藥品成為重要議題。
研究目的:本研究評估目前臺灣門診失眠患者領取zolpidem情形,分析影響失眠病人逛醫師行為的因素,以期能加以防治。
研究方法:採用次級資料庫進行研究分析,資料來源為2008-2010年全民健康保險資料庫,研究對象為2008年門診就醫紀錄中有失眠診斷且領取zolpidem即納入本研究,並將首次zolpidem處方日訂為指標日,自指標日後觀察24個月。本研究逛醫師行為的定義是病人在同病症同時期尋求2位(含)以上醫師就診,並領取重複處方。並進一步統計每位病人的逛醫師行為指標。
研究結果:研究對象共6,947人,無逛醫師行為者有5,295人(76.22%),以逛醫師行為領用zolpidem者有1,652人(23.78%)。有逛醫師行為者,逛醫師行為指標平均值為0.2(標準差0.23)。每增加1歲,產生逛醫師行為的機率為0.991倍、逛醫師行為指標降低0.6%;有慢性病者產生逛醫師行為是無慢性病者的1.232倍、逛醫師行為指標上升11.9%;疾病數每增加1項,產生逛醫師行為的機率為1.01倍;逛醫師行為指標增加8%,投保金額20,000-39,999元者,產生逛醫師行為的勝算比為投保金額0-19,999元者的1.367倍,投保金額40,000元以上者,產生逛醫師行為的勝算比為投保金額0-19,999元者的1.237倍;高社經地位產生逛醫師行為的勝算比為低社經地位的1.178倍、逛醫師行為指標降低10.4%;居住地平均每執業醫師服務人口數每增加1人,產生逛醫師行為的機率為0.9996倍,逛醫師行為指標增減少0.02%。
結論:逛醫師行為在臺灣是重要議題。傾向產生逛醫師行為者,為年齡越低、有慢性病者、疾病數越多、投保金額較低、高社經地位、醫療可近性較高者。有較高逛醫師行為指標,為年齡越低、有慢性病者、疾病數越多、投保金額較低、低社經地位、醫療可近性較高者。期望能建立處方整合平台,讓醫師、藥師皆可查詢,確實執行連貫性藥事照護,以減少以逛醫師行為領取zolpidem。
