Abstract
背景:心臟衰竭(Heart Failure, HF)是一種常見且逐年增加的疾病,尤其伴隨全球人口老化,已成為醫療系統負擔的重要問題。HF患者高達80%存在睡眠障礙(Sleep disturbances, SDs),特別是睡眠呼吸障礙(Sleep disorder breathing, SDB),對其健康狀態與心臟功能造成顯著影響。本研究旨在利用機器學習模型分析睡眠監測數據與心臟功能(左心室射出分率,LVEF)的關聯性,並藉由肺到手指循環時間(Lung-to-Finger Circulation Time, LFCT)作為新指標進行風險評估,以提供有效的非侵入性檢測工具。
方法:研究採回溯性分析,從衛生福利部雙和醫院睡眠中心收集173位患者資料,包括多項生理睡眠檢查(Polysomnography, PSG)和病史資料。患者依LVEF分為低風險(60–70%)與高風險(<60%或>70%)兩類。資料處理包括資料缺失值與異常值處理、標準化、正規化與平衡技術(Oversampling、Undersampling、SMOTE)。應用多種機器學習模型分析,包括:邏輯回歸(LG)、K近鄰演算法(K-NN)、支援向量機(SVM)、決策樹(DT)、隨機森林(RF)和XGBoost等。模型性能透過ROC曲線下面積(AUC)與混淆矩陣(Confusion matrix)進行比較。
結果:分析顯示,再透過不同資料平衡的技術下,可以優化模型驗證效能,另外邏輯回歸模型在不同資料平衡技術下表現最佳,AUC最高可達0.93。透過SHAP(Shapley Additive Explanations)分析特徵貢獻程度,BMI、心率、肺到手指循環時間(LFCT)與呼吸事件指標(呼吸暫停(Apnea)和低通氣(Hypopnea))對於模型預測提供了最多的預測貢獻。其中LFCT 數值較高在模型驗證中與心衰竭高風險具有正相關,反之數值較低對於風險低提供了正向貢獻,也對應了LFCT與心功能表現呈反向關係,表示LFCt可作為新的心臟風險評估因子。
結論:本研究利用機器學習模型與睡眠監測數據建立心臟功能風險評估工具,證實LFCT對於心臟衰竭風險評估可以為模型預測提供貢獻,其預測指標與臨床表現相同,LFCT與心功能表現呈反向關係,可作為新的生理標誌。此方法為非侵入性、低成本且適用於長期監測,有助於早期診斷與疾病管理。同時,本研究顯示不同增量技術可改善樣本數量不足與資料不平衡問題,並提升模型穩健性。未來建議進一步擴大樣本數,探索深度學習模型(如LSTM或CNN),並應用於可穿戴設備,促進遠程醫療與臨床實務應用。
