Abstract
研究目的:利用全民健康保險資料庫,以回溯性世代研究法分析digoxin是否會增加台灣末期腎臟疾病(end-stage renal disease, ESRD)須接受血液透析(hemodialysis, HD)治療病人死亡的風險,並評估病人潛在心臟疾病(心室上部心搏過速、心房顫動及撲動)、digoxin平均使用日劑量對於死亡發生的影響。
研究方法:本研究利用國家衛生研究院提供之全民健保百萬人承保抽樣歸人檔2000年至2010年之資料。主要評估:2001年至2008年間新診斷末期腎臟疾病需血液透析治療病人發生死亡之風險。次要評估:病人性別、年齡、潛在心臟疾病、共病、併用藥物、以及digoxin平均使用日劑量對病患發生死亡之相關性。
研究結果:存活分析結果顯示,校正性別、年齡、病人潛在心臟疾病、共病、以及併用藥物後發現,使用digoxin會顯著增加接受血液透析末期腎臟疾病病人發生死亡的風險(HR 1.238; 95% CI 1.074 to 1.425, p=0.003)。除此之外,使用digoxin患者的平均存活時間顯著比沒有使用之病患短10.9個月(62.9個月 vs. 72.8個月, p<0.001)。
結論:digoxin會顯著增加台灣接受血液透析末期腎臟疾病病人發生死亡的風險達23.8%,且縮短存活時間。
