Abstract
研究介紹:目前研究對於非癌症慢性疼痛病人長期使用鴉片類止痛劑的臨床效益與安全性評估尚未完全了解,致使此疼痛控管方式極具爭議性。現階段仍缺乏對於非癌症慢性疼痛病人長期使用鴉片類止痛劑或中重度疼痛治療的替代藥物與死亡率及醫療資源使用分析的關聯性調查,值得深入探討。

研究假說: 本研究假說為非癌症慢性疼痛病人族群使用長期鴉片類止痛劑相較中重度疼痛治療的替代藥物應有較高的風險發生死亡事件與使用相關醫療資源。

研究設計: 此為一篇使用活性對照組、以族群為基礎的世代研究。

研究方法: 本研究為一篇健保資料庫分析,探討2005至2011七年間所有新使用長期鴉片類止痛治療族群的發生率、盛行率與處方開立趨勢,並透過傾向分數以1:4 比例配對新使用中重度疼痛替代藥物案例作為活性對照組。本研究於一年、二年不同觀察期,以死亡事件為主要觀察項目;急診就醫、住院資源利用為次要觀察項目。本研究使用Cox比例風險模式計算校正後風險比,並使用多變項模型分析獨立變量因子。

研究結果: 本研究發現長期鴉片類止痛治療非癌症慢性疼痛之族群自2005至2011年間共有70%年盛行率的增加 (3 to 5.1 of 100,000 population)。Morphine、meperidine為最常使用於此非癌疼痛族群的鴉片類止痛劑,且多數meperidine針劑短期性使用於年輕層級。本研究共收錄158位長期鴉片類止痛劑與配對出601位新使用中重度疼痛替代藥物的慢性非癌疼痛病例。本研究發現長期鴉片類止痛劑使用族群於整體死亡率的校正後風險比(hazard ratio, HR),顯著較替代藥物組高 (HR, 3.88; 95% confidence interval [CI], 1.02-14.72)。另外,此長期鴉片類止痛劑使用族群於整體急診資源利用率的校正後風險比為1.66 (95% CI, 1.32-2.11)、慢性疼痛相關住院利用情形為1.88 (95% CI, 1.04-3.40)、呼吸疾病相關急診資源為2.73 (95% CI, 1.05-7.09),與替代藥物使用組相比,亦有顯著較高的事件發生風險。

結論: 我國長期使用鴉片類止痛劑於慢性非癌症疼痛病人的使用於近七年間逐漸盛行。然而,長期使用鴉片類止痛劑與使用疼痛替代藥物相比顯示較高的整體死亡率與整體急診資源使用率。此發現應做為臨床上長期鴉片類止痛劑使用於非癌症慢性疼痛的風險利益評估內容。
